摘 要:目的探讨不同剂量舒芬太尼在心脏瓣膜置换术患者全身麻醉气管插管应激反应。方法将2016年4月至2018年4月该院收治的90例心脏瓣膜置换术患者采用随机数字表法分为观察组和对照组,每组45例。对照组用2μg/kg剂量舒芬太尼,观察组用1μg/kg剂量舒芬太尼。比较两组患者心脏瓣膜置换术麻醉气管插管的效果,阿托品、多巴胺使用例数,麻醉前后患者平均动脉压、心率监测值、收缩压、舒张压、心率和舒张压乘积(RPP)等。结果观察组患者心脏瓣膜置换术麻醉效果明显高于对照组,阿托品使用例数明显低于对照组,差异均有统计学意义(P<0.05);两组患者麻醉前平均动脉压、心率监测值、收缩压、舒张压、RPP比较,差异均无统计学意义(P>0.05);观察组患者麻醉后平均动脉压、心率监测值、收缩压、舒张压、RPP均明显优于对照组,差异均有统计学意义(P<0.05)。结论1μg/kg剂量舒芬太尼在心脏瓣膜置换术麻醉气管插管时效果确切,可更好地维持动力学指标稳定,降低心肌耗氧量,减少阿托品、多巴胺的使用,且麻醉效果与2μg/kg剂量舒芬太尼相似。Objective To investigate the stress response of endotracheal intubation under general anesthesia in patients with cardiac valve replacement with different doses of sufentanil.Methods 90 patients with finger valve replacement in our hospital from April 2016 to April 2018 were randomly divided into groups.The control group was given 2 doses of chofentanyl per kg,and the observation group was given 1 dose of chofentanyl per kg.The results of anesthesia for endotracheal intubation were compared between the two groups.Examples of atropine and dopamine.Mean arterial pressure,heart rate.Results The anesthetic effect of cardiac valve replacement in the observation group was similar to that in the control group(P>0.05).The number of atropine use cases in the observation group was lower than that in the control group(P<0.05).There was no significant difference in mean arterial pressure,heart rate monitoring,systolic pressure,diastolic pressure and RPP between the two groups before anesthesia(P>0.05).The mean arterial pressure,heart rate monitoring,systolic pressure,diastolic pressure and RPP of the observation group were better than those of the control group(P<0.05).Conclusion 1μg/kg dose sufentanil in heart valve replacement when endotracheal intubation anesthesia effect is exact,can better maintain the dynamics index stability,reduce myocardial oxygen consumption and atropine,dopamine,and the effects of anesthesia and high dose sufentanil is similar.
