抗利尿激素分泌失调综合征三例报道并文献复习

摘　　要：抗利尿激素分泌失调综合征是指某些病理情况下内源性抗利尿激素持续不恰当分泌或活性增强,增加了肾集合管对水的通透性,从而导致体内水分过多、尿钠排泄增多以及稀释性低钠血症等一系列临床表现的一组综合征。本文总结了胜利油田中心医院3例抗利尿激素分泌失调综合征患者的诊断及治疗,并结合相关文献,探讨其发生的病因及诊断要点、治疗方法,为今后临床工作提供更多的诊断思路,避免漏诊、误诊。Syndrome of inappropriate antidiuretic hormone secretion(SIADH)is a disorder characterized by a series of clinical manifestations,such as excessive free water retained in the body,increased renal excretion of sodium and dilutional hyponatremia.The syndrome results from inappropriate,continued endogenous secretion or enhanced action of the antidiuretic hormone(ADH)by certain pathological conditions,which makes renal collecting duct be more permeable to water.We summarized the diagnosis and treatment of three patients with SIADH in Shengli Oilfield Central Hospital.Furthermore,based on the reviewing of the corresponding studies,we explored the etiology and key points in the diagnosis and management of this disease,in an attempt to present diagnostic approaches as many as possible to reduce the possibilities of missed diagnosis and incorrect diagnosis of SIADH.
