摘 要:目的:探讨"温里攻下法"中药对高脂血症性重症急性胰腺炎(HLSAP)大鼠肠黏膜通透性及甘油三酯的影响。方法:100只雄性SD大鼠按照随机数字表法,分为对照组、SAP组、HLSAP组、SAP治疗组和HLSAP治疗组共5组,每组20只,逆行胆胰管注射牛黄胆酸钠建立大鼠SAP模型,继续股静脉注射Triton Wr1339建立HLSAP模型,两治疗组于造模前2 h给予胰炎合剂灌胃,各组在设定时段中检测尿中乳果糖和甘露醇排泄率的比值(L/M)、血清甘油三脂(TG)和血清淀粉酶水平。结果:各时段SAP组及HLSAP组大鼠L/M逐渐升高,24 h、36 h达最高峰,该两组相比,HLSAP组L/M高于SAP组,但无显著性差异(P>0.05),与对照组相比差异具有统计学意义(P<0.05);HLSAP治疗组和SAP治疗组与SAP组及HLSAP组相比,在建模后时段12 h时段L/M无明显差异24 h、36 h时段SAP治疗组L/M明显低于SAP组(P<0.05),HLSAP治疗组明显低于HLSAP组(P<0.05)。术后各时段HLSAP组及HLSAP治疗组TG水平明显高于SAP组、SAP治疗组和对照组(P<0.05);24 h、36 h时段HLSAP治疗组TG下降水平最明显(P<0.05)。各时间段SAP组、HLSAP组、SAP治疗组及HLSAP治疗组血清淀粉酶水平明显高于对照组(P<0.05);24、36 h时段SAP治疗组及HLSAP治疗组血清淀粉酶水平分别较SAP组及HLSAP组有明显下降(P<0.05)。结论:高脂血症会增加SAP大鼠肠屏蔽功能障碍,胰炎合剂能改善大鼠肠黏膜通透性和肠屏障功能,可降低其TG水平。Objective To investigate the effect of“Wenli Gongxia”Chinese medicine method on intestinal permeability and triglyceride in rats with hyperlipidemic severe acute pancreatitis.Methods A total of 100 male SD rats were randomly divided into control group,severe acute pancreatitis group(SAP group),high fat blood hyperlipidemic severe acute pancreatitis group(HLSAP group)and SAP and HLSAP treatment group,(n=20,in each group).The urinary lactulose and mannitol exhaust discharge rate ratio(L/M)and serum triglyceride(TG)level were detected.Results The L/M of SAP group and HLSAP group increased gradually,and reached the highest peak after 24 h and 36 h.The L/M in HLSAP group was higher than that of SAP group,and there was no significant difference(P>0.05).The difference was statistically significant compared with the control group(P<0.05).In HLSAP treatment group and SAP treatment group,compared with SAP and HLSAP groups,after the modeling time and 12 h,no obvious difference was found between L/M.After 24 h and 36 h,the L/M levels in two treatment groups were obviously lower than those of SAP group and HLSAP group,difference had statistical significance(P<0.05).The SAP treatment group was significantly lower than SAP group(P<0.05).HLSAP treatment group was significantly lower than HLSAP group(P<0.05).In HLSAP group and HLSAP treatment group,the levels of TG were significantly higher than those in SAP group,SAP treatment group and contrast group(P<0.05).After 24 and 36 h,the levels of TG in each group were decreased significantly(P<0.05)and HLSAP treatment group decreased the level most obviously(P<0.05).The serum amylase levels in SAP group and HLSAP group were significantly higher than those in the control group at each time point(P<0.05).At 24 and 36 h after treatment,SAP treatment group and HLSAP treatment group were significantly lower than those of SAP group and HLSAP group(P<0.05).Conclusion Hyperlipidemia can aggravate the intestinal barrier dysfunction in severe acute pancreatitis,and the effect of Chinese her
