摘 要:目的:研究益肾解毒汤在巨噬细胞增殖活化方面对单侧输尿管梗阻(UUO)大鼠肾组织纤维化的影响。方法:将健康成年雄性SD大鼠48只随机分为假手术组、模型组、益肾解毒汤组和氯沙坦钾组,每组12只。假手术组大鼠游离左输尿管后,不结扎,分层缝合;其余3组大鼠均行UUO模型制备。益肾解毒汤组和氯沙坦钾组分别采用益肾解毒汤和氯沙坦钾每日灌胃1 mL,假手术组和模型组均予0.9%氯化钠溶液1 mL每日灌胃。2周后采用免疫组化染色检测大鼠肾组织中巨噬细胞表面标志物(-ED1)、增殖细胞核抗原(PCNA)以及诱生性一氧化氮合酶(iNOS)的表达,同时观察24 h尿蛋白定量。结果:模型组、益肾解毒汤组和氯沙坦钾组大鼠24 h尿蛋白定量分别为(10.70±1.02)mg、(7.52±0.78)mg、(8.32±0.57)mg,ED1的阳性表达面积分别为(65.25±12.05)μm2、(40.72±8.09)μm2、(51.76±7.85)μm2,PCNA的阳性表达面积分别为(73.2±9.43)μm2、(42.83±8.20)μm2、(53.49±8.27)μm2,iNOS的阳性表达面积分别为(74.4±9.10)μm2、(52.87±6.77)μm2、(67.50±8.13)μm2,均明显高于假手术组(P<0.05),益肾解毒汤组与模型组比较,24 h尿蛋白定量及ED1、PCNA、iNOS阳性表达面积均显著减少(P<0.05);而益肾解毒汤组大鼠24 h尿蛋白定量及ED1、PCNA、iNOS阳性表达面积均显著少于氯沙坦钾组。结论:益肾解毒汤能明显抑制ED1、PCNA、iNOS的表达,抑制巨噬细胞的增殖活化,有抗肾组织纤维化的作用。Objective To study the effect of Yishenjiedu decoction on the proliferation and activation of macrophages in rats with renal fibrosis induced by unilateral ureteral obstruction(UUO).Methods A total of 48 rats were randomly divided into four groups(n=12):sham operation group,model group,Yishenjiedu decoction group,and Losartan Potassium group.The left ureter of the sham operation group was not ligated after being dissociated,while the UUO model was used in the other 3 groups.Yishenjiedu decoction group and the Losartan Potassium group were respectively given intragastric administration of 1mL Yishenjiedu decoction and Losartan Potassium daily,while the sham operation group and the model group were given 0.9%sodium chloride solution.After 2 weeks,immunohistochemical staining method was used to detect the expression of ED1,PCNA and iNOS in renal tissue;and meanwhile the 24 hours(24 hrs)urinary protein level was observed.Results The 24 hrs urine protein of the model group,Yishenjiedu decoction group and Losartan potassium group were(10.70±1.02)mg,(7.52±0.78)mg,and(8.32±0.57)mg,respectively.They were significantly higher than the sham operation group(P<0.05).Compared with the model group,the 24 hrs urine protein quantification and the positive expression areas of ED1,PCNA and iNOS in Yishenjiedu decoction group were significantly decreased(P<0.05).Comparing the Yishenjiedu decoction group to the Losartan potassium group,the 24h urine protein and the positive expression areas of ED1,PCNA and iNOS in the Yishenjiedu decoction group were significantly lower than that in the Losartan potassium group.Conclusion Yishenjiedu decoction can inhibit the proliferation and activation of macrophage macrophage,and alleviate kidney fibrosis.
