尖吻腹蛇血凝酶致高热、呼吸困难1例  

One case of hyperpyrexia and dyspnea induced by haemocoagulase agkistrodon

摘　　要：1例48岁男性患者因"体检发现血小板减少2个月"入院,行脾切除+贲门周围血管离断+脾肾静脉分流术,先后给予抗感染、抑酸、护肝、止血等治疗,术后第1天使用尖吻蝮蛇血凝酶过程中突发寒战、呼吸困难、高热。医生排除疾病因素,临床药师结合患者药品使用情况考虑为尖吻腹蛇血凝酶所致严重过敏反应,转入重症医学科监护治疗,停用可疑药物,其他药物继续使用,给予对症处理,患者体温、氧饱和度逐渐恢复正常,次日转回外科继续原治疗,未再使用尖吻腹蛇血凝酶,上述不适症状也未再出现。A 48-year-old male patient was admitted to the hospital because of thrombocytopenia for 2 months.Splenectomy,peripheral blood vessel detachment from the cardia,and spleen-kidney venous shunt were performed.The anti-infection,acid suppression,liver protection and hemostasis were given successively.After the using of haemocoagulase agkistrodon at the first day after the surgery,the patient appeared chills,dyspnea and high fever.The disease factors were ruled out by doctors,and clinical pharmacists considered this possible allergic reaction may be due to haemocoagulase agkistrodon after investigating the medication of this patient.The patient was transferred to intensive care unit.The suspicious drug was stopped;however,other drugs were used.After symptomatic treatment,the body temperature and oxygen saturation of this patient gradually returned to normal.The next day,the patient was back to surgical ward for further treatment.Haemocoagulase agkistrodon was not used,and the above reactions did not occur again.
