摘 要:目的探讨叶酸联合维生素B12对伴有高同型半胱氨酸血症(HHcy)的轻度认知功能障碍(MCI)患者的疗效。方法伴有HHcy的MCI患者100例,随机分为治疗组和对照组各50例,另设健康组50例。治疗组给予叶酸10 mg·d-1、维生素B12片500μg·d-1口服;对照组给予安慰药口服;健康组不进行药物干预。观察24周。治疗前、治疗24周末分别测定3组血浆同型半胱氨酸(Hcy)、叶酸、维生素B12水平,评定简易精神状态检查表(MMSE)、蒙特利尔认知评估量表(Mo CA)。结果治疗后,治疗组血浆Hcy较治疗前下降[(17.96±8.72)和(27.30±8.78)μmol·L-1],叶酸升高[(17.23±6.70)和(8.59±5.97)ng·m L-1]、维生素B12升高[(335.03±78.35)和(234.87±70.87)pmol·L-1](均P<0.01),MMSE、Mo CA量表评分无明显变化;对照组血浆Hcy、叶酸、维生素B12及MMSE量表评分均无明显变化,Mo CA量表评分下降(P<0.01)。结论叶酸联合维生素B12能有效降低伴有HHcy的MCI患者的血浆Hcy,延缓MCI患者认知水平下降。Objective To investigate the effect of folic acid combined with vitamin B 12 in the treatment of mild cognitive impairment(MCI)in patients with hyperhomocystinemia(HHcy).Methods In total,100 cases of MCI with HHcy were randomly divided into treatment group and control group(50 cases in each group),and healthy group also had 50 cases.The treatment group was given folic acid 10 mg·d-1 and vitamin B 12 tablet 500μg·d-1;the control group was given the consolation medicine orally;no drug intervention was given in the health group.They were observed for 24 weeks.The levels of plasma Hcy,folic acid and vitamin B 12 were measured in the three groups before and after the 24 weeks of treatment.The simple mental state Checklist(MMSE)and the Montreal cognitive assessment scale(MoCA)were evaluated.Results After the treatment,the plasma Hcy in the treatment group was significantly decreased as compared with that before the treatment[(17.96±8.72)vs.(27.30±8.78)μmol·L-1],folic acid increased[(17.23±6.70)vs.(8.59±5.97)ng·mL-1],vitamin B 12 elevated[(335.03±78.35)vs.(234.87±70.87)pmol·L-1](all P<0.01).There was no obvious change in the score of MMSE and MoCA.There was no significant change in plasma Hcy,folic acid,vitamin B 12 and MMSE scores in the control group,and the score of MoCA scale decreased(P<0.01).Conclusion Folic acid combined with vitamin B 12 can effectively reduce plasma Hcy in patients with HHcy accompanied by MCI and delay cognitive decline in MCI patients.
