摘  要:目的探讨腹腔镜联合胆道镜下液电碎石治疗胆总管难取性结石的疗效。方法回顾性分析我院2015年8月至2018年3月期间胆总管难取性结石患者86例,其中采用腹腔镜联合胆道镜下液电碎石治疗胆总管难取性结石43例作为微创组,另外开腹取石43例患者为开腹组。结果微创组的43例患者均顺利完成胆总管结石的治疗,平均手术时间(150±25)min。术中出血平均(40±5)mL。所有患者均治愈出院,3例患者因行一期缝合后于术后出现胆漏,2例经充分引流后自然闭合,1例患者于术后第三天再次手术行T管引流术,术后治愈出院。结论在熟练掌握腹腔镜、胆道镜技术的前提下,掌握好患者的适应症,对胆总管难取性结石患者采用腹腔镜联合胆道镜下液电碎石治疗,是一种可供选择的安全的微创治疗方法。Objective To investigate the curative effect of laparoscopic combined with cholangioscopic liquid electrolithotripsy in the treatment of Intractable choledocholithiasis of the choledochal duct.Methods The clinical data of 43 patients with intractable choledocholithiasis treated by laparoscopic combined with hydroelectric lithotripsy from November 2015 to March 2018 in our hospital were retrospectively analyzed.Results 43 patients successfully completed the treatment of bile duct stones,the average operation time was(150±25)min.Average intraoperative bleeding was(40±5)mL.All patients were cured and discharged.Three patients had bile leakage after the first stage of suture,two patients were naturally closed after full drainage,and one patient was re-operated for T-tube drainage on the third day of surgery.After the surgery,the patient was cured and discharged.Conclusion Under the premise of mastering laparoscopic and cholangioscopic techniques.it was a safe and minimally invasive treatment method to master the indications of patients and to treat patients with gallbladder stones that were difficult to obtain by laparoscopic combined with cholangioscopy.
