摘  要:目的:探析对心脏手术患儿应用右美托咪定药物麻醉后对机体应激反应及血流动力学的影响。方法:收治心脏手术治疗先天性心脏病患儿60例,随机分为两组。对照组给予0.9%氯化钠注射液,观察组给予持续泵入右美托咪定。比较两组静脉置管给药前(T0)、给药后5 min(T1)、开胸骨后(T2)、体外循环前(T3)、体外循环结束(T4)时应激反应和血流动力学变化。结果:观察组T1、T2、T3、T4时点心率低于对照组,差异有统计学意义(P<0.05)。观察组T1、T2、T3、T4时点平均动脉压、中心静脉压及心排血量优于对照组,差异有统计学意义(P<0.05)。结论:右美托咪定可降低心脏手术患儿的应激反应及血流动力学指标。Objective:To explore the effect of dexmedetomidine on stress response and hemodynamics in children undergoing cardiac surgery.Methods:60 cases of children with congenital heart disease treated with cardiac surgery were divided into two groups randomly.The control group was given 0.9%Nacl solution,and the observation group was given a continuous infusion of dexmedetomidine,and the changes of stress response and hemodynamics were compared between the two groups at T0(before intravenous administration),T1(5 min after administration),T2(after the sternum opening),T3(before cardiopulmonary bypass),and T4(the end of extracorporeal circulation).Results:The heart rate of the observation group at T1,T2,T3 and T4 were lower than those of the control group,the difference was statistically significant(P<0.05).The mean arterial pressure,central venous pressure and cardiac output in the observation group at T1,T2,T3 and T4 were better than those of the control group,the difference was statistically significant(P<0.05).Conclusion:Dexmedetomidine could reduce the stress response and hemodynamic changes in children undergoing cardiac surgery.
