血清H-FABP水平在不同程度左室射血分数保留的心力衰竭患者中的相关性研究

摘　　要：目的探讨血清H-FABP水平在不同程度左室射血分数保留的心力衰竭患者中的临床应用价值。方法选取2013年3月1日~2014年2月20日住院的左室射血分数保留的心力衰竭患者72例作为研究对象,按照纽约心脏病学会(New York Heart Academy,NYHA)心功能分级标准将其分成Ⅱ级组24例,Ⅲ级组25例和Ⅳ级组23例。观察心功能Ⅱ、Ⅲ、Ⅳ级各组H-FABP、NT-proBNP水平并进行比较。结果四组患者的既往年龄、性别、病因构成、既往高血压病、高脂血症、心率、血压、饮酒史、糖尿病史、吸烟史、既往药物治疗史等基础资料分布均衡,差异无统计学意义(P>0.05);经Spearman相关性分析显示,从Ⅱ级组到Ⅳ级组,H-FABP与NT-proBNP水平呈逐渐增高趋势(r=0.649、r=0.907),差异有统计学意义(P<0.01),任意两亚组间比较,差异有统计学意义(P<0.05),经Pearson相关分析提示:H-FABP与NT-proBNP显著正相关。结论在判断左室射血分数保留的心力衰竭患者病情严重程度及心功能分级方面H-FABP和NTproBNP具有类似价值,临床上应该加以重视。Objective To discuss clinical value of the level of H-FABP in serum in the patients with different degree of Heart Failure with preserved ejection fraction(HF-PEF).Methods 72 cases of the hospitalized patients with HF-PEF,from March 1st,2013 to February 20th,2014,were chosen and grouped into 3 subgroups(24 cases in classⅡgroup,25 cases in classⅢgroup and 23 cases in classⅣgroup)by NYHA cardiac function classification.The levels of H-FABP and NT-proBNP in classⅡ,ⅢandⅣgroups were observed and compared.Results By balanced test,there was no significant difference in the basic data of four groups'patients(including age,sex,fac tor,hypertension,hyperlipemia,heart rate,blood pressure,drinking history,diabetes history,smoking history and medical therapy history)(P>0.05).Analysis of Spearman's correlation showed that there was an increasing tendency of the levels of H-FABP and NT-proBNP from classⅡgroup to classⅣgroup,(r=0.649、r=0.907),and there was statistical significance between the two groups(P<0.01).There was significant difference in the comparison of random two groups(P<0.05).Analystis of Pearson's correlation showed that there was a significant positive correlation.Conclusion H-FABP has the same value as NT-proBNP in the judgement of disease severity and cardiac function classificati
