摘 要:目的:通过口服葡萄糖耐量试验(OGTT)探讨佛山地区糖耐量正常人群与初诊2型糖尿病患者血糖、胰岛素和胰高血糖素样肽-1(GLP-1)的特点,以阐明佛山地区正常人群和初诊2型糖尿病患者的GLP-1与胰岛b细胞功能的关系。方法:按纳入标准和排除标准选取正常人群和初诊2型糖尿病患者56例,其中正常人18例,初诊2型糖尿病患者38例,通过口服葡萄糖耐量试验(OGTT)观察空腹及OGTT中各时间点的血糖、血浆胰岛素和GLP-1水平的变化。结果:糖耐量正常组与T2DM组胰岛素、GLP-1和血糖的分泌呈单时相;糖耐量正常组胰岛素、GLP-1及血糖的达峰时间早于T2DM组;两组的胰岛素及GLP-1值在OGTT后30min及60min时差异有统计学意义,两组的血糖值在所有观察时段差异均有统计学意义。结论:2型糖尿病患者餐后胰岛素及GLP-1的达峰时间较正常糖耐量组延后,二者的分泌量峰值均较正常糖耐量组降低,这可能提示新诊断2型糖尿病患者已现早期胰岛b细胞功能受损。Objective:To investigate the characteristics of blood glucose,insulin and glucagon-like peptide-1(GLP-1)in the general population and newly diagnosed type 2 diabetic patients in Foshan by oral glucose tolerance test(OGTT).Relationship of GLP-1 and islet B-cell function between the general population and newly diagnosed type 2 diabetic patients were explored.Methods:According to the inclusion criteria and exclusion criteria,56 patients were enrolled,including 18 normal subjects and 38 newly diagnosed type 2 diabetes.OGTT was applied to observe fasting and blood glucose,plasma insulin and GLP-1 levels at different time.Results:The secretion of insulin,GLP-1 and blood glucose in the normal group and T2DM group were single phase.The peak time of insulin,GLP-1 and blood glucose in the normal group was earlier than those in the T2DM group.There were differences in insulin and GLP-1 between both groups at 30 minutes and 60 minutes after OGTT.The blood glucose levels in the two groups showed statistically significant differences at all observation periods.Conclusion:The peak time of postprandial insulin and GLP-1 in patients with type 2 diabetes is later than that in the normal group.The peak of secretions is lower than the normal group,which may suggest that patients with newly diagnosed type 2 diabetes suffered from impaired islet B-cell function early.
