老年高血压患者认知功能障碍影响因素的调查分析

摘　　要：目的:探讨老年高血压患者认知功能障碍的影响因素。方法:选取2017年3-6月在本院体检中心查体的老年高血压患者189例,使用简易智能状态检查量表(MMSE)进行认知功能评估,并收集相关资料,根据得分将老年高血压患者分为认知功能正常组102例和认知功能障碍组87例。比较两组年龄、性别、文化程度、病程、体质指数(BMI)、收缩压、舒张压、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FPG)、糖化血红蛋白(Hb AIc)、血尿酸、血肌酐、血尿素氮的差异,采用Logisti回归分析筛选认知功能障碍的影响因素。结果:与认知功能正常组相比,认知功能障碍组文化程度较低(文化程度大学及以上:28.74%VS 51.96%,P<0.01),病程[(23.8±6.7)年VS(15.8±7.5)年,P<0.01]长,Hb AIc[(6.02±0.88)%VS(5.19±0.79)%,P<0.01]较高,Logistic回归分析结果显示文化程度、高血压病程、Hb AIc是老年高血压患者发生认知功能障碍的危险因素,OR(95%CI)分别为3.213(1.672~6.211)、3.651(1.571~8.522)和3.559(1.369~6.542),均P<0.01。结论:老年高血压患者认知功能障与文化程度、病程、Hb AIc密切相关,对高血压进行及早有效干预,积极控制Hb AIc水平,对保护老年高血压认知功能有着重要意义。Objective:To investigate factors affecting of cognitive dysfunction in elderly hypertensive.Methods:189 patients were evaluated by MMSE,all patients were divided into the normal cognitive function group(102 cases)and the cognitive dysfunction group(87 cases).Results:Compare education level(higher education:28.74%VS 51.96%,P<0.01),duration of disease[(23.8±6.7)years VS(15.8±7.5)years,P<0.01],HbAIc[(6.02±0.88)%VS(5.19±0.79)%,P<0.01]in groups,the differences were statistically significant.Logistic regression analysis showed that factors affecting of cognitive dysfunction in elderly hypertensive include education level,duration of hypertension,and HbAIc.OR(95%CI)was 3.213(1.672~6.211),3.651(1.571~8.522),and 3.559(1.369~6.542),respectively,P<0.01.Conclusion:The cognitive dysfunction of elderly hypertensive is closely related to education;course of disease and HbAIC.It is significant to protect the cognitive function by early effective intervention and active control of HbAIC level.
