摘　　要：目的: 探讨临床药师在肾功能不全患者抗凝药物治疗期间使用抗真菌药物的作用。方法: 临床药师参与1例慢性血栓栓塞合并肾功能不全患者的药物治疗过程中发生出血事件的分析,协助医师调整药物治疗方案,并监测出血症状及凝血常规指标,分析发生出血症状的原因。结果: 医师采纳临床药师建议,患者出血症状好转。结论: 临床药师参与特殊人群华法林个体化剂量的制订和调整,保证了患者的用药安全、有效。Objective:To explore the role of clinical pharmacists participating in the treatment of a patient with renal insufficiency with anticoagulant and antifungal drugs.Methods:Clinical pharmacists participated in the analysis of bleeding events during the drug treatment of the patient with chronic thromboembolism and renal insufficiency,assisted physicians to adjust therapy plan and monitored the symptoms of hemorrhage and the routine indicators of blood coagulation,and analyzed the causes of occurrence of bleeding symptom.Results:Physicians adopted the suggestion of clinical pharmacists and the bleeding symptom of the patient was improved.Conclusion:Clinical pharmacists participate in the formulation and adjustment of warfarin individualized dosage in special population,which can ensure the safety and effectiveness of drug use.
