腹腔镜胆总管切开胆道镜取石T管引流术与开腹手术治疗结石临床效果差异

摘　　要：目的分析腹腔镜胆总管切开胆道镜取石T管引流术与开腹手术治疗结石临床效果差异。方法选择2016年8月~2017年6月在本院就诊的胆总管结石患者80例,经双盲随机法分为观察组和对照组,各40例,对照组施行开腹手术,观察组施行腹腔镜胆总管切开胆道镜取石T管引流术,分析两组患者治疗效果。结果两组患者手术均顺利完成,成功率均100.00%;观察组手术时间、术中出血量、术后通气时间以及住院时间均低于对照组(P<0.05);观察组及对照组术后5 d内检测TBIL水平、AST以及ALT水平差异无统计学意义;观察组治疗后7 d SAS及SDS评分均显著低于对照组及治疗前(P<0.05);观察组治疗后7 d QOL生活质量显著各维度评分均显著低于对照组(P<0.05)。结论经腹腔镜胆总管探查治疗胆总管结石可获得良好的治疗效果。Objective To analyze the difference of clinical effect between laparoscopic choledocholithotomy and choledochoscope T tube drainage and laparotomy for calculus.Methods 80 cases of choledocholithiasis in our hospital from August 2016 to June 2017 were divided into observation group and control group,40 cases in each group.The control group was operated on laparotomy.The observation group was treated with laparoscopic choledochoscope and choledochoscope T tube drainage,and the treatment effect of the two groups was analyzed.Results All the two groups were successfully completed with a successful rate of 100.00%.The operation time,the amount of bleeding,the time of postoperative ventilation and hospitalization of observation group were lower than those of the control group(P<0.05).There was no significant difference in the level of TBIL,AST and ALT in the observation group and the control group at 5 d after operation.The scores of AS and SDS were significantly lower than those in the control group and before the treatment(P<0.05),and the scores of the quality of life at 7 d QOL in the observation group were significantly lower than those in the control group(P<0.05).Conclusion Laparoscopic common bile duct exploration for choledocholithiasis can achieve good results.
