宫腔镜治疗剖宫产术后子宫瘢痕妊娠的疗效探讨

摘　　要：目的总结宫腔镜手术治疗剖宫产术后子宫瘢痕妊娠(CSP)的临床疗效。方法以2014年4月~2017年7月本院收治的剖宫产术后CSP患者60例患者为研究对象,遵循随机抽样法分参照组和分析组,每组30例。参照组予以常规清宫术治疗,分析组则执行宫腔镜手术治疗,比较两组治疗效果。结果从两组患者临床治疗时间、术中出血量、人绒毛膜促性腺激素(β-HCG)恢复时间、月经恢复时间、住院时间等指标水平可见,分析组明显短于参照组,差异具有统计学意义(P<0.05);对比两组患者治疗后满意度,分析组总满意率为93.33%(28/30),参照组为70.00%(21/30),差异具有统计学意义(P<0.05);术后并发症统计显示,分析组总发生率6.67%(2/30)明显低于对照组23.33%(7/30),差异具有统计学意义(P<0.05)。结论对于剖宫产术后子宫瘢痕妊娠的临床治疗,以宫腔镜手术治疗可获得更高疗效收益,利于减少术中出血量,缩短治疗时间与患者康复时间,提升满意度,降低术后并发症发生率,具有重要临床推广应用价值。Objective To summarize the clinical effect of hysteroscopic operation on uterine scar pregnancy(CSP)after cesarean section.Methods From April 2014 to July 2017,60 CSP patients treated in our hospital from April 2014 to July 2017 were divided into reference group(30 cases)and analysis group(30 cases)according to random sampling method.The control group was treated with conventional hysteroscopy and the analysis group with hysteroscopy.Results The clinical treatment time,intraoperative bleeding volume,recovery time of human chorionic gonadotropin(β-HCG),menstrual recovery time and hospitalization time were measured.It was found that the analysis group was significantly shorter than the reference group,the difference was statistically significant(P<0.05),compared with the two groups,the total satisfaction rate of the analysis group was 93.33%(28/30),and the control group was 70.00%(21/30),the difference was statistically significant(P<0.05),and the postoperative complications were statistically significant(P<0.05).The total incidence of 6.67%(2/30)in the analysis group was significantly lower than that in the control group 23.33%(7/30),and the difference was statistically significant(P<0.05).Conclusion The clinical treatment of uterine scar pregnancy after cesarean section can be obtained by hysteroscopy.It is helpful to reduce the amount of intraoperative bleeding,shorten the time of treatment and patients'rehabilitation,improve satisfaction and reduce the incidence of postoperative complications,which has important clinical application value.
