微创经皮肾输尿管镜取石术的临床方法及应用效果研究

摘　　要：目的探讨微创经皮肾输尿管镜取石术的临床方法及应用效果。方法选取本院2013年11月~2017年11月收治的100例输尿管上段结石患者,随机分为即常规组与研究组,每组50例。常规组实施尿路输尿管镜取石术治疗;研究组实施微创经皮肾输尿管镜取石术治疗。结果研究组术后3 d结石清除率96.00%、术后30 d结石清除率100.00%,显著高于常规组82.00%、90.00%(P<0.05);研究组术中失血量(36.5±5.7)ml、术后住院天数(3.6±1.3)d显著少于常规组(51.2±6.1)ml、(7.0±1.6)d(P<0.05);两组手术时间[(65.5±6.3)min、(65.4±6.2)min]比较差异无统计学意义;研究组手术并发症2.00%明显少于常规组18.00%(P<0.05)。结论微创经皮肾输尿管镜取石术能够有效清除结石,减轻手术创伤程度,且并发症少,值得临床进一步推广。Objective To discuss the clinical method and application effect of minimally invasive percutaneous nephrolithotomy.Methods A total of 100 patients with upper ureteral calculi admitted to our hospital from November 2013 to November 2017 were randomly divided into a routine group and a research group,with 50 patients in each group.The conventional group was treated by ureteroscopic lithotomy;the study group was treated by minimally invasive percutaneous nephrolithotomy.Results The stone clearance rate in the study group was 96.00%after operation 3 days,100.00%after operation 30 days,which was significantly higher than that of the routine group of 82.00%and 90.00%(P<0.05);the amount of blood loss(36.5±5.7)ml and postoperative hospital days(3.6±1.3)d in the study group were significantly less than that of the conventional group(51.2±6.1)ml and(7.0±1.6)d(P<0.05),There was no significant difference between the operation time of the two groups(65.5±6.3)min and(65.4±6.2)min;the operative complication of the study group(2.00%)was obviously less than that of the routine group(18.00%)(P<0.05).Conclusion Minimally invasive percutaneous nephrolithotomy can effectively remove stones,reduce the degree of surgical trauma,and have fewer complications,it is worthy of further clinical promotion.
