摘 要:目的:探讨补肾强脊颗粒联合塞来昔布对强直性脊柱炎(AS)成纤维细胞抗骨化作用及BMP/Smad信号通路的影响。方法:选取疼痛科收治的AS患者100例,采用随机数字表法分为两组。对照组口服塞来昔布胶囊;观察组口服塞来昔布胶囊加用补肾强脊颗粒。结果:治疗后两组临床疗效各指标均较前显著改善(P<0.05),且观察组在BASDAI积分(13.29±5.13)分、BADFI积分(21.48±6.47)分、疼痛积分(1.56±0.54)分、晨僵时间(18.57±7.93)分,均较对照组显著改善(P<0.05)。两组Schober试验、扩胸度、指地距均较前显著改善(P<0.05),且观察组在Schober试验(4.42±1.14)较对照组(3.84±1.21)显著改善(P<0.05)。观察组SPARCC评分(6.27±3.26)分,对照组(7.35±2.45)分均较前显著改善(P<0.05)。两组BMP受体水平均较前显著改善(P<0.05),且观察组BMP-Ⅱ受体水平(0.042±0.003)显著降低于对照组(0.087±0.004),(P<0.05)。治疗后两组Smad水平均较前显著改善(P<0.05),且观察组Smad1水平(0.806±0.048)、Smad5水平(0.413±0.078)显著降低于对照组(P<0.05)。结论:补肾强脊颗粒联合塞来昔布通过抑制成纤维细胞中BMP/Smad信号通路的活化来延缓AS患者向成骨型分化,发挥抗骨化作用。Objective:To investigate the effect of Bushen Qiangji particles combined with celecoxib on the antiossification of AS fibroblast and BMP/Smad signaling pathway.Methods:Select our hospital 100 cases of the AS patients,using the random number table method,divided into two groups.Control group oral celecoxib capsules,observation group oral celecoxib capsules combined with Bushen Qiangji particles.Results:After treatment,the clinical efficacy of the two groups was significantly improved(P<0.05),and the observation group of BASDAI score(13.29±5.13),BADFI score(21.48±6.47),pain score(1.56±0.54),and morning stiffness time(18.57±7.93).Significantly improved compared with the control group(P<0.05).The two groups Schober test,chest expansion,finger-to-ground distance were significantly improved(P<0.05),and the observation group in the Schober test(4.42±1.14)compared with the control group(3.84±1.21)was significant improvement(P<0.05).SPARCC score(6.27±3.26)points in the observation group,(7.35±2.45)points in the control group were significantly improved(P<0.05).BMP receptor levels in the two groups were significantly improved than before(P<0.05),and the observation group BMP-II receptor level(0.042±0.003)was significantly lower than the control group(0.087±0.004),(P<0.05).After treatment,the two groups Smad levels were significantly improved(P<0.05),and observed the level of Smad1 in the group was(0.806±0.048)and the Smad5 level was(0.413±0.078),which was significantly lower in the control group(P<0.05).Conlusion:Bushen Qiangji particles combined with celecoxib can inhibit the activation of BMP/Smad signaling pathway in fibroblasts and delay the differentiation of AS patients into osteogenesis and play an anti-ossification role.
