大黄红藤消炎方治疗重症急性胰腺炎临床疗效及对PCT、sICAM-1影响分析

摘　　要：目的:探讨大黄红藤消炎应用于重症急性胰腺炎(SAP)患者辅助治疗的临床价值。方法:在入院治疗SAP患者中选取70例分为参照组(n=35,常规治疗)、研究组(n=35,加用大黄红藤消炎方治疗),观察患者血淀粉酶、血清PCT及sICAM-1等相关指标改善情况。结果:经治疗,研究组患者sICAM-1及PCT水平均明显低于参照组(P<0.05)。研究组WBC计数、血淀粉酶水平也明显低于参照组(P<0.05),且两项指标恢复正常水平时间均短于参照组(P<0.05)。治疗后,研究组患者炎性因子(IL-6、IL-8及TNF-α、CRP)水平均有明显降低,且均显著低于参照组(P<0.05),研究组患者腹痛、腹胀、肠鸣音等症状、体征改善时间及住院天数均明显短于参照组(P<0.05)。结论:在常规治疗基础上从中医学角度给予SAP患者大黄红藤消炎方治疗,可有效降低炎性因子,使患者的胃肠动力获得更好改善,加快相关临床症状、体征改善速度,进而缩短患者住院时间。Objective:To evaluate investigate the clinical value of Dahuang Hongteng Xiaoyan in the treatment of severe acute pancreatitis(SAP).Methods:70 cases of SAP patients were divided into the reference group(n=35,routine treatment)and the research group(n=35,combined with Dahuang Hongteng Xiaoyan prescription),the improvement of blood amylase,serum PCT and sICAM-1 were observed.Results:After treatment,the levels of sICAM-1 and PCT in the study group were significantly lower than those in the reference group(t=27.234、9.614,P<0.05),the WBC count and blood amylase level in the study group were also significantly lower than those in the reference group(P<0.05),the two indicators returned to normal levels were shorter than the reference group(P<0.05).After treatment,inflammatory factors(IL-6,IL-8 and TNF-alpha,CRP)levels in the study group were significantly reduced,and it was significantly lower than the reference group(P<0.05).The abdominal pain,abdominal distention,bowel sounds and other symptoms,signs improvement time and hospitalization days in the study group were significantly shorter than those in the reference group(P<0.05).Conclusion:On the basis of conventional treatment,SAP patients were treated with Dahuang Hongteng Xiaoyan prescription from the perspective of traditional Chinese medicine,can effectively reduce the inflammatory factors,to improve the gastrointestinal motility of the patients,to speed up the improvement of clinical symptoms and signs,and shorten the length of stay in patients.
