气管切开患者机械通气期间安全吸痰的临床观察

摘　　要：目的探析气管切开患者机械通气期间安全吸痰的方法、技巧及临床效果。方法收集2015年4月至2017年4月期间在我院ICU行气管切开机械通气的110例患者的一般资料,按就诊编号分组,单号纳入开放组(n=55),进行开放式吸痰;双号纳入密闭组(n=55),接受密闭式吸痰。将操作过程中两组不良反应展开对比,并观察吸痰前后患者监护指标的变化。结果密闭组吸痰后气道堵塞、气道损伤及剧烈咳嗽伴痰液喷出总发生率与开放组相比下降,差异有统计学意义(P<0.05);吸痰后5min密闭组患者收缩压、心率与开放组相比降低,指尖血氧饱和度上升,差异有统计学意义(P<0.05)。结论与开放式吸痰相比,在气管切开患者机械通气期间采用密闭式吸痰有助于稳定患者各项生命体征,减少不良反应,改善心肺功能。Objective:To explore the method,technique and clinical effect of safe suction in patients with tracheotomy during mechanical ventilation.Methods:110 cases of patients were Collected from April 2015to April 2017 in our hospital ICU tracheotomy mechanical ventilation as the research object,and the pathological data were analyzed retrospectively.All patients were divided into two groups according to treatment number,patients with serial number were included in the open group(n=55)and given open-ended aspiration;Patients with double number were included in the closed group(n=55),received closed suction.The adverse reactions of the two groups were compared during the operation of suctioning,and the changes of the monitoring indexes of the patients before and after suctioning were observed.Results:The incidence of airway obstruction,airway injury,and cough with sputum in the closed group were significantly lower than those in the open group(P<0.05).Compared with the open group,the systolic blood pressure and heart rate increased in the closed group 5 min after suctioning,and the difference was statistically significant(P<0.05).Conclusion:Compared with open suction,mechanical ventilation during tracheostomy in patients with closed suction can help stabilize the vital signs,reduce adverse reactions,and improve heart and lung function.
