居家护理新模式在重度肺动脉高压患者出院后管理中的应用

摘　　要：[目的]探讨居家护理新模式对重度肺动脉高压患者出院后管理的效果。[方法]该研究招募2014年11月至2016年12月我科连续收治重度肺动脉高压(PAH)患者20例(平均年龄19±8.3岁,男性20±3.4;女23±3.2岁),所有患者均需要在出院后继续使用胰岛素泵(丹纳泵)经皮下持续输注曲前列尼尔注射液治疗PAH。建立基于移动通讯技术的患者出院后治疗的居家护理新模式,采用多媒体、短信等形式,出院前对患者进行健康宣教,指导使用丹纳泵安全管理,出院后观察不良反应及调整治疗。[结果]20例患者出院后随访36-930天(150±30.6)。20例患者临床症状较住院时改善,胸闷气短症状好转,活动耐力改善以及NT-BNP值显著下降(P<0.05)。无显著不良反应。[结论]基于移动通讯技术的居家护理模式有效改善重度PAH患者出院后管理,改善患者症状,提高治疗依从性和生命质量。[Objective]To explore the effect of a self-care model on management in patients with pulmonary arterial hypertension(PAH)after discharge.[Methods]20 patients(mean age 19±8.3 years,male 20±3.4;female 23±3.2 years)with PAH were enrolled into the study from November 2014 to December 2016.The patients were administered by subcutaneous continuous infusion of TreprostinilInjection with insulin pump.A self-care managemental model based on model mobile technology was built.The symptom,activity tolerance,and adverse effect were compared between in-hospital and out-of-hospital..[Results]Twenty patients were followed up for 36-930 days(150±30.6)and clinical symptoms were improved compared with hospitalization,including chest tightness,shortness of breath,endurance and NT-BNP values(p<0.05).Insulin pump was safely used in the 20 cases and there were no significant adverse reactions.[Conclusion]The self-care model based on mobile technology,which relieving the symptoms,improved the management of patients with PAH after discharge.
