不同胆囊切除术对患者术后细胞免疫与胃肠功能的影响比较

摘　　要：目的:观察比较不同胆囊切除术对患者术后细胞免疫与胃肠功能的影响。方法:选取2016年1月-2017年12月本院胆囊切除术患者80例。根据手术方式的不同分为A组(开腹手术组)和B组(腹腔镜手术组),各40例。比较两组患者的胃肠功能恢复情况、手术前后的细胞免疫及胃肠动力相关血液指标。结果:B组肠鸣音恢复时间、排气时间及排便时间均短于A组,比较差异均有统计学意义(P<0.05);术前,两组患者的细胞免疫及胃肠动力相关血液指标比较,差异均无统计学意义(P>0.05);术后,B组患者的细胞免疫指标及胃肠动力相关血液指标均优于A组,比较差异均有统计学意义(P<0.05)。结论:腹腔镜胆囊切除术对患者术后细胞免疫与胃肠功能的影响显著优于开腹手术,更有助于本类患者的术后康复,因此更为适用于本类患者。To observe and compare the influence of different cholecystectomy for the postoperative cellular immunity and gastrointestinal function of patients.Method:A total of80patients with cholecystectomy in our hospital from January2016to December2017were selected.According to the differences of operation modes,they were divided into A group(laparotomy group)and B group(laparoscopic operation group),40cases in each group.The recovery situation of gastrointestinal function,cellular immunity and gastrointestinal motility related blood indexes before and after operation of two groups were compared.Result:The recovery time of bowel sound,exhaust time and defecation time of B group were all shorter than those of A group,the differences were statistically significant(P<0.05).Before operation,the cellular immunity and gastrointestinal motility related blood indexes of two groups were compared,the difference were not statistically significant(P>0.05).After operation,the cellular immunity indexes and gastrointestinal motility related blood indexes of B group were all better than those of A group,the differences were statistically significant(P<0.05).Conclusion:The influence of laparoscopic cholecystectomy for the postoperative cellular immunity and gastrointestinal function of patients are significantly better than those of open cholecystectomy,and it is more helpful to the postoperative rehabilitation of patients,so it is more suitable for this kind of patients
