ICU机械通气并肺部感染老年患者行纤维支气管镜肺灌洗的治疗效果分析

摘　　要：目的分析重症监护病房(ICU)机械通气并肺部感染老年患者应用纤维支气管镜肺灌洗治疗对临床疗效、降钙素原(PCT)及细胞因子的影响。方法分析该院于2015年4月-2017年4月收治的ICU机械通气并肺部感染80例老年患者资料,按照不同治疗方案分成两组,将应用ICU常规方式治疗40例患者作为对照组,将与纤维支气管镜肺灌洗联合治疗40例患者作为观察组,比较两组临床疗效、PCT、C反应蛋白(CRP)及血气指标情况。结果观察组总有效率比对照组高,且PCT(P=0.000)、CRP(P=0.000)均比对照组低;观察组动脉血二氧化碳分压(PaCO_2)(P=0.000)、动脉血氧分压(PaO_2)(P=0.000)均比对照组优。结论 ICU机械通气并肺部感染老年患者应用纤维支气管镜肺灌洗治疗能够改善其临床症状,且可优化患者PCT、CRP和血气指标水平,值得推广。Objective To analyze the intensive care units(ICU)mechanical ventilation and pulmonary infection in elderly patients with fiberoptic bronchoscopy lung lavage therapy on the clinical efficacy and effect of calcitonin and cytokines.Methods Clinical data of 80 elderly patients with pulmonary infection treated with mechanical ventilation in ICU from April 2015 to April 2017 was analyzed.All the patients were divided into two groups according to different treatment regimens,40 patients treated with ICU routine treatment as control group,while the other 40 patients treated with combination treatment of fiberoptic bronchoscopy lung lavage as the observation group.Then compare the clinical efficacy,procalcitonin,C-reactive protein and blood gas between the two groups.Results In the observation group,the total effective rate is higher than that in control group,and procalcitonin and C reactive protein levels were lower than that in control group(P<0.05);the observation group PaCO2 and PaO2 were better than that in control group(P<0.05).Conclusion Fiberoptic bronchoscopy lung lavage therapy can improve the clinical symptoms of elderly patients treated with mechanical ventilation while complicated with pulmonary infection in ICU.Meanwhile,the procalcitonin and C reactive protein and blood gas level of patients were improved.It is worthy of promotion.
