医药分开综合改革对某传染病专科医院运营的影响分析

摘　　要：目的通过分析北京市医药分开综合改革对我院运行情况的影响,总结经验,提出应对措施,以期为传染病医院平稳度过改革期,借助医药卫生体制改革契机实现自身发展提供借鉴。方法采取自身同期对照法,对比我院改革前1年(2016年)及改革实施当年(2017年)不同时段内的运行数据,并对跨期变化情况进行配对t检验或秩和检验,分析医药分开综合改革政策对我院运行指标的影响。结果医药分开综合改革政策在我院实施效果明显,医疗服务量呈现下降趋势,医疗服务能力提高,医疗收入结构得到了一定改善,但在药品耗材管控、推进分级诊疗、提高患者满意度等方面还需重点关注和加强。结论在深化医药卫生体制改革过程中,传染病专科医院应注重加强内涵建设,以患者为中心,改善和提升医疗服务质量,提高运行效率,落实精细化管理;要发挥医疗联合体的作用,形成资源共享;积极响应医药卫生体制改革,争取政策支持等。Objective By analyzing the influence of the comprehensive medical reform in Beijing,which separated medical service from drug sales,on the operation of our hospital,we summarized our experiences and put forward countermeasures,so as to provide references for the smooth transformation of infectious disease hospitals during the reform.Methods We compared the operational data in our hospital one year before the reform(in 2016)with those in the reform year(2017).The changes across these two years were analyzed by paired t test or rank sum test,so as to evaluate the influence of the new policies on the operation indicators of our hospital.Results Significant outcome has been achieved in the medical and health system reform in our hospital.The quantity of medical service declined,the medical service capability increased,and the medical income structure has been improved.However,problems still existed in terms of control of drug/medical supplies,pursuit of hierarchical medical service,and patient satisfaction,which should be addressed and improved.Conclusions During the implementation of the medical and health system reform,infectious disease hospitals should make efforts to strengthen their connotation construction,emphasize the patient-centered concept,improve medical service quality,raise operation efficiency,and implement fine management.In addition,medical consortium should play an efficient role in resource-sharing.Finally,hospitals should actively respond to the reform of medical and health system and seek policy support.
