曲安萘德联合维生素B_(12)硬膜外腔注射治疗腰椎间盘突出症的临床观察

摘　　要：目的研究曲安萘德联合维生素B12硬膜外腔注射治疗腰椎间盘突出症的临床疗效,旨在更好的为腰椎间盘突出症患者治疗提供参考依据。方法选取甘肃省庆阳市人民医院在2015年3月—2017年3月期间收治的80例腰椎间盘突出症患者,收集患者的临床资料进行回顾性分析。根据治疗方式的不同将患者分为对照组和观察组,每组各40例,对照组患者采用曲安奈德1 mL(40 mg)+2%的利多卡因6 mL+0.9%氯化钠8 mL,共组成混合液15 mL治疗,研究组患者采用曲安萘德1 ml(40mg)+维生素B121 ml(0.5 mg)+2%利多卡因6 ml+0.9%氯化钠8 ml,共组成混合液16 ml治疗。观察不同治疗方式对两组患者临床疗效、疼痛缓解率和不良反应情况发生率的影响。结果研究组患者优良率为90.00%,对照组患者优良率为72.50%,两组患者比较存在差异(P<0.05);治疗1周后研究组患者疼痛缓解率明显高于对照组(P<0.05),而在治疗2周后以及3周后两组患者疼痛缓解率比较无差异(P>0.05);两组患者在治疗期间出现的并发症有月经紊乱、兴奋失眠、恶心呕吐、神经异常和心悸胸闷等症状出现,而研究组患者并发症发生率为25.00%,对照组患者并发症发生率为47.50%,研究组患者并发症发生率明显低于对照组(P<0.05)。结论曲安萘德联合维生素B12能明显提高患者的临床疗效,缓解患者疼痛症状,且治疗后并发症少,值得在临床上推广使用。OBJECTIVE To investigate the clinical efficacy of triamcinolone combined with vitamin B12 by epidural injection treatment of lumbar disc herniation to provide reference for better treatment of lumbar disc herniation.METHODS 80 cases with lumbar disc herniation in our hospital from March 2015 to March 2017 were selected.The clinical data of the patients were retrospectively analyzed.According to the different treatment,the patients were divided into two groups with 40 cases in each group.The control group was treated with a mixture of 15mL including triamcinolone 1 mL(40 mg)+2%lidocaine 6mL+0.9%sodium chloride 8mL,and the study group was treated with a mixture of 16ml including triamcinolone acetonide 1 ml(40 mg)+B12 1ml(0.5mg)+2%lidocaine 6 ml+0.9%sodium chloride 8ml.The effect of different treatment methods on the clinical efficacy,pain relief rate and incidence of adverse reactions in the two groups were observed and compared.RESULTS The excellent rate in the study group was 90.00%,and the excellent rate in the control group was 72.50%,and there was significant differences between the two groups(P<0.05).After 1 weeks of treatment,the remission rate of pain in the study group was significantly higher than that in the control group(P<0.05),and at 2 weeks and 3 weeks after treatment the remission rate of pain was no significant difference between the two groups(P>0.05).The complications during treatment in two groups of patients included menstrual disorders,insomnia,nausea and vomiting,excited nerve abnormalities,palpitation and chest tightness.The complication rate in the study group was 25.00%,and the rate in the control group was 47.50%.The complications rate in the study group was significantly lower than that in the control group(P<0.05).CONCLUSION The triamcinolone combined withvitamin B12 could significantly improve the clinical efficacy of patients,relieve pain symptoms with fewer complications after treatment.It was worth promoting in clinical use.
