淋巴细胞主动免疫治疗复发性流产前后血清免疫组学比较

摘　　要：目的:通过比较淋巴细胞主动免疫治疗不明原因的复发性流产(URSA)前后血清免疫组学表达,评价淋巴细胞免疫治疗的临床意义。方法:选取54例封闭抗体阴性BA(-)的复发性流产患者为研究对象,观察治疗后的BA表达和妊娠情况,通过流式细胞术和免疫比浊法检测外周血T细胞亚群以及外周血免疫球蛋白的变化。结果:相对于治疗前,治疗后BA(-)组的患者中T细胞亚群CD_3^+、CD_8^+亚群数量增加,CD_4^+及CD16^+CD56^+亚群比例下降,外周血免疫球蛋白IgM和IgG表达下降,差异均存在统计学意义(P<0.05);IgA变化没有显著性差异(P>0.05);BA阳性(+)组患者的所有指标与治疗前相比均无统计学差异(P>0.05)。阳性BA(+)组妊娠成功率达91.4%,BA(-)组妊娠成功率为36.8%,两组妊娠成功率间差异存在统计学意义(P<0.05)。结论:淋巴细胞主动免疫治疗复发性流产效果显著,T细胞亚群的变化可能有利于妊娠,封闭抗体治疗值得临床推广。Objective:To compare the serum immunohistochemistry before and after Lymphocyte active immunotherapy in unexplained recurrent spontaneous abortion(URSA)and normal pregnant,in order to evaluate the clinical significance of Lymphocyte immunotherapy.Methods:A total of 65 blocking antibody(BA)negative expression URSA patients were performed in our hospital.To observe the pregnancy progress,T-lymphocyte subsets and peripheral blood immunoglobulin changes after BA treatment.Results:Compared with before treatment,the number of CD 3+and CD 8+subpopulations of CD 4+and CD 16+CD 56+subsets decreased,and the expression of IgM and IgG in peripheral blood decreased in patients with BA-after treatment.Significance(P<0.05);there was no significant difference in IgA changes(P>0.05).There was no significant difference in all indexes of patients in the BA+group compared with treatment before(P>0.05).The success rate of pregnancy in the BA+group was 91.4%,and the success rate in the BA-group was 36.8%,and there was a statistically significant difference between the two groups(P<0.05).Conclusion:The change of T-lymphocyte subsets could be good for pregnancy.Lymphocyte active immunotherapy is significant in URSA,which worth clinical promotion.
