结核感染T细胞斑点试验及结核抗体检测对肺外结核病的诊断价值

摘　　要：目的:探讨结核感染T细胞斑点试验(T-SPOT.TB)及结核抗体对肺外结核病患者的诊断价值。方法:选择疑似肺外结核病患者50例纳入本次研究。分别对所有患者进行结核抗体实验以及T-SPOT.TB实验检查,分析T-SPOT.TB实验与结核抗体实验的检测结果,对比T-SPOT.TB实验与结核抗体实验对于肺外结核病的检测价值。结果:T-SPOT.TB实验的检测结果显示,阳性患者42例,占84.00%;阴性8例,占16.00%。结核抗体实验的检测结果显示,阳性患者40例,占80.00%,阴性患者10例,占20.00%。T-SPOT.TB实验对于肺外结合病的诊断灵敏度为88.37%,特异度为42.86%,准确度为82.00%,均分别明显高于结核抗体实验的81.40%、28.57%、78.00%,差异有统计学意义(P<0.05)。结论:T-SPOT.TB实验与结核抗体实验相比,在诊断肺外结核患者的价值上具有更好的作用,但为进一步提升诊断准确率,应考虑将T-SPOT.TB实验与结核抗体实验联合进行诊断,值得临床重视。Objective:To study the diagnostic value of T cell spot test of tuberculosis infection and tuberculosis antibody in patients with pulmonary tuberculosis.Methods:50 patients with suspected tuberculosis were enrolled in this study.Tuberculosis antibody test and T-SPOT.TB test were performed in all patients,analysis of experimental results of T-SPOT.TB test and tuberculosis antibody test,compared the value of T-SPOT.TB test and tuberculosis antibody test for the detection of pulmonary tuberculosis.Results:The results of T-SPOT.TB test showed that 42 cases were positive,accounting for 84.00%;Negative 8 cases,accounting for 16.00%.The test results of tuberculosis antibody test showed that 40 cases were positive,accounting for 80.00%,Negative patients in 10 cases,accounting for 20.00%.The diagnostic sensitivity of T-SPOT.TB test for the diagnosis of pulmonary external binding was 88.37%,the specificity was 42.86%,the accuracy was 82.00%,which were significantly higher than the 81.40%,28.57%,78.00%,,respectively,the difference was statistically significant(P<0.05).Conclusion:Compared with that of the experiment and T-SPOT.TB tuberculosis antibody test,in the value of diagnosis of pulmonary tuberculosis patients with better effect,but for further enhance the diagnostic accurate rate and should be considered joint diagnosis of T-SPOT.TB test and tuberculosis antibody test,it is worth clinical attention.
