摘　　要：听神经瘤是起源于第八对颅神经的良性肿瘤,大多数来源于前庭神经的鞘膜,来源于蜗神经者极少。前庭上神经来源的肿瘤比前庭下神经来源者术后听力的保留效果好,蜗神经来源者术后保留听力的效果最差。术前准确判断听神经瘤的起源对于手术进路的选择以及预估术后听力的保存均具有指导意义。本文通过介绍1例蜗神经起源的听神经瘤,并通过文献复习,分析听力学检查、内耳MRI以及冷热实验、前庭诱发肌源性电位(VEMP)对于听神经瘤的起源所具有的术前评估作用。Acoustic neuroma is a benign tumor originated from the eighth cranial nerve,most of which from the sheath of the vestibular nerve and few from the cochlear nerve.Tumor origin has a great impact on post-operative hearing preservation.Superior vestibular nerve originated tumors have better postoperative hearing preservation potentials compared with tumors from the inferior vestibular nerve,whereas cochlear nerve originated tumors have the worst potentials.Preoperative assessment of tumor origin may provide guidance for both surgical approach and post-operative hearing preservation.In this article,we summarize pre-operative assessment using inner ear MRI,caloric tests and vestibular evoked muscle potentials(VEMP),and report post-operative hearing preservation outcomes in a case of cochlear nerve originated acoustic neuroma while reviewing relevant literatures.
