摘　　要：目的:探究医用磁共振成像(MRI)系统弥散加权成像(DWI)联合血清糖链抗原125(CA125)和人附睾蛋白4(HE4)检测对卵巢囊腺癌的临床诊断价值。方法:回顾性分析86例卵巢肿瘤患者的临床资料,按照术后病理学诊断结果将其分为卵巢良性肿瘤组(52例)和卵巢囊腺癌组(34例)。所有患者术前均行DWI检查,同时测定血清CA125及HE4水平。分析比较单一检测及三者联合检测对卵巢囊腺癌诊断的价值。结果:卵巢囊腺癌实性部分的表观弥散系数(ADC)值低于卵巢良性肿瘤患者,血清CA125和HE4水平均高于卵巢良性肿瘤组,组间比较差异具有统计学意义(t=6.69,t=19.61,t=15.39;P<0.05),而囊性部分的ADC值两组间比较差异无统计学意义(t=0.79,P>0.05)。DWI联合血清CA125及HE4检出卵巢囊腺癌的阳性率最高(占91.2%),明显高于DWI、CA125及HE4三者的单独检测。结论:DWI联合血清CA125和HE4检测在卵巢囊腺癌的诊断中具有重要的临床价值,较单一检测可明显提高诊断灵敏度及特异度。Objective:To investigate the clinically diagnostic value that MR diffusion weighted imaging(DWI)combined with detections of serum carbohydrate antigen(CA125)and human epididymis protein 4(HE4)for ovarian cystadenocarcinoma.Methods:The clinical data of 86 patients with ovarian cystadenocarcinoma were retrospectively analyzed and they were divided into benign ovarian tumor group(52 cases)and ovarian cystadenocarcinoma group(34 cases)according to results of postoperatively pathological diagnosis.All of patients received preoperative DWI examination and the detections of serum CA125 and HE4.And then,the diagnostic value of single detection and combined detections of three items for ovarian cystadenocarcinoma were further compared and analyzed.Results:The ADC value of the solid part of ovarian cystadenocarcinoma group was significantly lower than that of benign ovarian tumor group,while the former's serum CA125 and HE4 levels were significantly higher than that of the later(t=6.69,t=19.61,t=15.39,P<0.05),respectively.On the other hand,the difference of cystic part between the two groups were no significant(t=0.79,P>0.05).In addition,the positive rate of combined detection of three items was highest(91.2%),and it was obviously higher than single examination of DWI,CA125 and HE4,respectively.Conclusion:This kind of combined detection of DWI with serum CA125 and HE4 has important clinical value in the diagnosis of ovarian cystadenocarcinoma.Compared with single examination,its diagnostic sensitivity and specificity are obviously improved,and it is worthy in clinical application.
