摘  要:目的:了解抗阻运动干预对糖尿病前期(IGR)人群胰岛素抵抗(IR)的影响并与有氧运动干预结果进行比较,为控制或逆转IGR、预防糖尿病(DM)提供理论和实践依据。方法:随机对照实验。筛选出的研究对象随机分成抗阻运动组(R组,23人)、有氧运动组(A组,26人)和对照组(C组,21人),有氧和抗阻运动干预均为每周3次、每次50 min中等强度,各组分别进行12周的运动干预和/或健康教育后随访其血糖、胰岛素和体成分,并计算其胰岛素抵抗指数2(HOMA2-IR)和β细胞功能指数2(HOMA2-β)。结果:R组干预后HOMA2-IR均值下降18. 31%(P <0. 01),与C组相比有非常显著性差异; A组干预后HOMA2-IR均值下降8. 34%(P <0. 05),与C组相比有显著性差异; C组HOMA2-IR均值小幅上升6. 23%(P=0. 578)。根据血糖异常类型分类统计后发现:在IGT人群中,HOMA2-IR均值A组和R组分别下降了9.58%和12. 42%,A组仅为R组的2/3; HOMA2-β未因干预而产生显著性变化。体成分:R组体脂百分比(FAT%)下降较多的同时瘦体重增加;而A组FAT%非常显著下降的同时瘦体重也显著下降。结论:有氧和抗阻运动均能改善IGR人群的外周IR,且抗阻运动的干预效果更突出。Objective:To understand the effects of resistance exercise intervention on insulin resistance(IR)in pre-diabetic(IGR)population and compare them with aerobic exercise interventions with the same frequency,duration,and intensity so as to provide theoretical and practical basis for controlling or reversing IGR and preventing diabetes(DM).Methods:Random controlled single-blind experiments.The subjects selected according to the inclusion and exclusion criteria were randomly divided into the resistance exercise group(Group R,23),the aerobic exercise group(Group A,26),and the control group(Group C,21)by computer software.The aerobic and resistance exercise interventions of moderate intensity were performed three times a week,50 minutes each time.The intervention subjects did not know the presence of the other groups.After receiving 12 weeks of exercise intervention and/or health education,each group was tested for blood glucose,insulin and body compositions,and their insulin resistance index 2(HOMA2-IR)and beta cell function index 2(HOMA2-β)were also calculated.Results:The mean value of HOMA2-IR in Group R decreased by 18.31%(P<0.01)after intervention,and there was a significant difference compared with Group C(P<0.01).The mean value of HOMA2-IR in Group A decreased by 8.34%after intervention(P<0.05),there was a significant difference compared with Group C(P<0.05);the mean value of HOMA2-IR in Group Cincreased slightly by 6.23%(P=0.578).According to the classification of blood glucose abnormalities,it was found that in the IGT population,the mean value of HOMA2-IR in Group A and group C decreased by 9.58%and 12.42%respectively,with Group A being only 2/3 of Group R;there was no significant change in HOMA2-βafter intervention.Body compositions:The percentage of body fat(FAT%)in Group R decreased more while the lean body mass increased;the FAT%in Group A decreased significantly,and the lean body mass also decreased significantly.Conclusion:Both aerobic and anti-resistance exercise can improve peripheral insulin resista
