摘 要:目的探讨听力初筛未通过婴幼儿联合应用畸变产物耳声发射(DPOAE)和自动听性脑干反应(Automated auditory brainstem responses,AABR)进行听力复筛的意义。方法运用AccuScreen听力筛查仪,对2016年6月至2017年7月扬州地区听力初筛未通过的300例(600耳)婴幼儿同时进行DPOAE和AABR联合听力复筛,其中任何一项未通过者均在3月龄时进行听性脑干反应、听性稳态反应等诊断性检查。结果在600耳中,500耳(双耳240例,单耳20例)DPOAE和AABR均通过;63耳(双耳24例,单耳15例)DPOAE和AABR均未通过,其中,有38耳(单耳6例,双耳16例)诊断为分泌性中耳炎,7耳(双耳3例,单耳1例)诊断为大前庭水管综合征,38耳(双耳13例,单耳12例)诊断为感音神经性聋;DPOAE通过而AABR未通过的4耳(2例双耳)诊断为小儿听神经病;DPOAE未通过而AABR通过的33耳(双耳3例,单耳27例)中,10耳(双耳3例,单耳4例)诊断为分泌性中耳炎,23耳(单耳23例)听力正常。结论 AABR和DPOAE联合听力复筛,可早期发现听力障碍患儿,有利于对其进行早期诊断、早期干预。Objective To explore the significance of combined application of distortion product otoacoustic emission(DPOAE)and automatic auditory brainstem response(AABR)in infants failed the in first hearing screening in Yangzhou.Methods From June 2016 to July 2017,300 cases(600 ears)failed in the first hearing screening were undertaken DPOAE and AABR using the AccuScreen hearing screening instrument.Further diagnostic exammations including ABR and ASSR were applied to those failed again either in DPOAE or AABR at the age of 3 months.Results In the 60 ears(300 cases),500 ears passedboth DPOAE and AABR in second hearing screening and moved into the follow-up period;63 ears failed either in DPOAE or in AABR in second hearing screening of which,38 ears were diagnosedwith secretory otitis media,7 ears were diagnosed with large vestibular aqueduct syndrome,and 38 ears were diagnosed with sensorineural deafness.4 ears passed DPOAE but failed in AABR,and were diagnosed with auditory neuropathy;33 ears passed AABR but failed in DPOAE,of which,10 ears were diagnosed with secretory otitis media,23 ears were confirmed with normal hearing.Conclusion The combination of AABR and DPOAE helps to early detection of hearing impairment in infants.
