摘　　要：目的探讨3~6岁学龄前儿童听力筛查的有效方法及其听力损失发生情况。方法应用瞬态诱发耳声发射(TEOAE)对5 602例3~6岁学龄前儿童进行听力筛查,初筛未通过者2周内行TEOAE复筛,复筛未通过者1月内进行畸变产物耳声发射(DPOAE)、声导抗、听性脑干反应(ABR)、听性稳态反应(ASSR)等听力学诊断。结果初筛通过5 277例(94.20%,5 277/5 602),未通过325例(5.80%,325/5 602);复筛323例,通过148例,未通过175例,其中耵聍栓塞23例(4.10%,23/5 602);接受听力学诊断132例,听力正常58例,异常74例,听力损失检出率为1.32%(74/5 602),包括:分泌性中耳炎65例(1.16%):其中单侧轻度传导性聋38例,双侧轻度传导性聋21例,单侧中度传导聋5例,双侧中度传导聋1例;感音神经聋9例(0.16%):其中双侧轻度聋3例,单侧轻度聋2例,单侧中度聋1例,单侧全聋2例,一侧轻度聋、一侧中度聋1例。结论本组学龄前儿童听力损失检出率为1.32%,以传导聋及轻度、单侧感音神经聋为主;TEOAE检查可用于学龄前儿童听力筛查。Objective To investigate the effective method of hearing screening in kindergarten children,and to understand the hearing loss of preschool children.Methods A total of 5 602 cases of 3~6-year-old children were screened by transient evoked otoacoustic emission(TEOAE)in kindergarten.Those who failed the screening were re-screened within 2 weeks by TEOAE.Audiologic diagnosis was made in those who failed the re-screening.Results 5 277 cases(94.2%,5 277/5 602)passed and 325 cases(5.8%,325/5 602)failed in the first screening step,148 cases passed and 175 cases including 23 cases of cerumen embolism failed in the re-screening step.Only 132 cases were diagnosed and 74 cases(1.32%,74/5 602)of hearing impairment were diagnosed in which 65 cases(1.16%)of secretory otitis media and 9 cases(0.16%)of sensorineural hearing loss.Conclusion The incidence of hearing impairment in preschool children is high,and it is necessary to carry out regular objective hearing screening.TEOAE is a feasible method in kindergarten children's hearing screening.
