摘 要:目的探讨民航飞行员听力损失分级的相关影响因素。方法整群抽样2017年度诊断为听力损失的民航飞行员886例,随机抽取同期体检听力正常的飞行员573例,共计1 459例纳入研究;对可能影响民航飞行员听力损失分级的因素,包括:高密度脂蛋白(HDL)、甘油三脂(TG)、低密度脂蛋白(LDL)、谷丙转氨酶(ALT)、脉压差、是否2型糖尿病、血管病变、脂肪肝、总飞行时间、年龄、性别等,进行等级Logistic回归分析;控制年龄因素后,应用偏相关分析进一步探索影响因素间的内在关联。结果等级Logistic回归分析结果示,总飞行时间越长(估计值=1.16E-005,P=0.031)、HDL(估计值=-0.217,P=0.043)和脉压差越低(估计值=-0.012,P=0.001)以及患有血管病变者(估计值=-0.207,P=0.024)的民航飞行员听力损失分级越严重;当年龄作为控制变量时,总飞行时间与舒张压的偏相关系数r=0.017,二者间无统计学相关性(P=0.509)。结论注重个体化听力防护,合理调控血压、血脂水平以及有效降低血管病变发病率可阻断影响因素间的交互作用,有益于民航飞行员的听力健康。Objective To investigate the possible influence factors related to hearing loss classification in civil aviation pilots.Methods By cluster sampling of 886 civil aviation pilots diagnosed with hearing loss in 2017 and random sampling of 573 pilots with normal hearing during the same period,a total of 1 459 cases were recruited in this study.We applied ordinal Logistic regression to study the factors that may affect the classification of hearing loss in civil aviation(HDL,TG,LDL,ALT,pulse pressure,type 2 diabetes,vascular disease,fatty liver,total flight time,age and gender).Age was set as a controlling factor.Partial correlation analysis was applied to further explore the intrinsic correlation between the influence factors.Results Ordinal Logistic regression results show that the longer the total flight time(Estimate=1.16E-005,P=0.031),the lower the HDL level(Estimate=-0.217,P=0.043)as well as the pulse pressure(Estimate=-0.012,P=0.001)and complications of vascular diseases(estimate=-0.207,P=0.024),the more serious the hearing loss classification was;Partial correlation coefficient between total flight time and diastolic pressure levels was 0.017 when age was taken as the control variable.There was no statistical correlation between the two factors(P=0.509).Conclusion Individualized hearing protection,reasonable regulation of blood pressure as well as blood lipids and effective reduction of the incidence of vascular diseases can block the interaction between influence factors,which is beneficial to the hearing health of civil aviation pilots subjects.
