手术联合围手术期嗓音训练治疗声带息肉的疗效分析

摘　　要：目的探讨手术联合围手术期嗓音训练对声带息肉患者嗓音恢复的疗效。方法 96例声带息肉患者随机分成A组(手术治疗,50例)和B组(手术联合嗓音训练,46例),B组术前和术后2周分别接受嗓音训练4周;随机选取48例无明显嗓音疾患的正常人作为正常对照组,采用Xion声学分析软件对正常对照组及A、B两组患者术前、术后6周的嗓音基频微扰(jitter)、振幅微扰(shimmer)、嗓音障碍指数(dysphonia severity index,DSI)、谐噪比(harmonic to noise ratio,HNR)、最长发声时间(maximum phonation time,MPT)进行测试,比较三组结果。结果与治疗前比较,治疗后A组和B组的jitter、shimmer值降低,DSI、HNR及MTP值升高,差异有统计学意义(P<0.05);治疗后B组的jitter、shimmer值低于A组,DSI、HNR及MTP值高于A组,差异有统计学意义(P<0.05),且治疗后B组的jitter、shimmer、MTP、DSI及HNR值与对照组比较差异无统计学意义(P>0.05)。结论手术联合围手术期嗓音训练更有利于改善声带息肉患者术后的嗓音质量。Objective To explore the clinical effect of surgical combined perioperative voice training for vocal recovery of early vocal polyps.Methods A total of 96 patients with early vocal cord polyps were randomly divided into group A(simple surgical treatment,50 cases)and group B(surgery combined with voice training,46 cases),group B received 4 weeks of voice training before and 2 weeks after surgery,48 people with no obvious voice disorder were used as the normal control group.The acoustic analysis software of Xion was used to conduct acoustic analysis of the voice in all groups before and 6 weeks after surgery,and the results were compared.Results Compared with pre-treatment outcomes,the jitter and shimmer value in group A and group B were reduced after treatment,the DSI,HNR and MTP were increased,and the difference was statistically significant(P<0.05).After training,jitter,shimmer value in group B were lower than those of in group A,DSI,HNR and MTP value were higher than those of in group A,the difference was statistically significant(P<0.05).There were no significanly differences in post-treatment outcomes of group B compared with control group(P>0.05).Conclusion Surgery combined with voice training has a good effect on improving the quality of voice in early vocal polyp patients.
