摘 要:目的探讨窄带成像技术(narrow band imaging,NBI)应用于喉乳头状瘤诊断的可行性。方法以2015年6月至2017年6月就诊的疑似喉乳头状瘤患者72例为研究对象,所有患者均行电子鼻咽喉镜普通白光模式和NBI模式检查,比较两种模式对病灶的检出率,以病理结果为诊断金标准,比较电子鼻咽喉镜NBI模式和普通白光模式对喉乳头状瘤诊断的准确性、敏感性和特异性。结果 72例患者共检出98处病灶,其中,电子鼻咽喉镜NBI模式发现98处病灶,而白光模式仅发现70处病灶,检出率为71.43%(70/98)。98处病灶病理确诊为乳头状瘤82处,非乳头状瘤16处;与病理诊断比较,NBI模式对喉乳头状瘤诊断的准确性、敏感性和特异性分别为90.82%(89/98)、95.12%(78/82)和68.75%(11/16);普通白光模式分别为74.49%(73/98)、79.27%(65/82)和50.00%(8/16),NBI模式诊断喉乳头状瘤的准确性和敏感性明显高于普通白光模式,且差异有统计学意义(均为P<0.05),两种模式电子喉镜诊断喉乳头状瘤的特异性无统计学差异(P>0.05)。结论电子鼻咽喉镜NBI模式可提高喉乳头状瘤的检出率及诊断喉乳头状瘤的准确性和敏感性。Objective To discuss the afflication feasibility of narrow band imaging in laryngeal papilloma.Methods Form June 2015 to June 2017,72 patients suspected with laryngeal papilloma were selected.All the patients were undertaken electronic nasopharyngolaryngoscopy both in common white light and NBI modes.The pathological results were set as the gold standard for the diagnosis of laryngeal papilloma.To compare the accuracy,sensitivity and specificity of electronic nasopharyngolaryngoscopy in white light mode and NBI modes in diagnosis of laryngeal papilloma.Results In all the 72 patients,98 lesions were found in the NBI model,while only 70 lesions in the white light mode,with the detection rate of 71.43%(70/98);the former was more sensitive than the latter(P<0.05).The accuracy,sensitivity and specificity of NBI mode in diagnosis of laryngeal papilloma were 90.82%(89/98),95.12%(78/82)and 68.75%(11/16),whereas of the white light mode were 74.49%(73/98),79.27%(65/82)and 50.00%(8/16).The accuracy and sensitivity of NBI mode in diagnosis of laryngeal papilloma were significantly higher than of the white light mode(P<0.05).However the specificity between the two mode was not significantly different(P>0.05).Conclusion The NBI mode of electronic nasopharyngolaryngoscope not only can improve not only the detection rate,but also the accuracy and sensitivity in diagnosis of laryngeal papilloma.
