摘　　要：目的:在科层制理论基础上,了解北京城市公立医院科层管理现状,探究其中的问题,为公立医院管理提供智力支持。方法:通过目的抽样的方法,对北京6所公立三级甲等医院的16名医院管理者、30名医务人员、48名患者进行个人深入访谈。结果:医院的组织结构主要为"党委领导下院长负责制"下的"直线职能型"结构。医院的科层制特征中,积极因素前3位是分工明确、工作人员遵守制度和规则、较高的团队合作水平。患者总体上表示北京大医院的医患信任程度和就诊体验较好,职称高的医务人员可以诊治更多的疾病,医疗专业的精细化分工使诊疗效果更可靠。结论:进一步完善制度章程,推进可操作性强的部门规章;构建扁平化组织,保证信息传递畅通;加强横向部门之间的沟通,为患者提供便捷服务。Objectives:To understand the current situation of the management of public hospitals,find out problems,and to provide intelligence support for public hospital management with the theory of bureaucracy.Methods:By means of the sampling method,16 hospital administrators,30 medical staff and 48 patients in 6 public tertiary hospitals in Beijing were selected to have an in-depth interview.Results:The organizational structure of the hospital is mainly the"linear functional"structure under the leadership of the president of the party committee.Dissecting the characteristics of the hospital,the top three positive factors is to have a clear division of responsibilities,staff abide by the rules and regulations,a higher level of team work.It shows that doctor-patient trust and clinic experience of patients in Beijing is relatively good.Those who have high professional title can carry out more treatment.Refine management and professional clinical care classification makes better diagnosis and treatment.Conclusions:It is considered to complete rules and regulations,promote operable rules and regulations,build a flat organization to ensure the smooth flow of information,strengthen communication between horizontal departments to provide convenience for patients.
