代谢综合征患者胰岛素抵抗与血浆醛固酮相关性的分析

摘　　要：目的探讨代谢综合征患者胰岛素抵抗及血浆醛固酮水平的关系。方法93例MS患者,根据MS不同组成成分分为MS、MS合并2型糖尿病及MS合并高血压组,所有受试者均行3h口服葡萄糖耐量试验及卧立位醛固酮试验,以及稳态模型公式计算胰岛素抵抗指数(HOMA-IR)。结果在ALD、INSAUS、HOMA-IR与HOMA-IR≥2.18各项指标中,MS各亚组中存在更高比例的患者数量,且均高于对照组(P<0.05)。另外,MS1组各项指标占比均高于MS2、MS3组(P<0.05)。MS2组占比略高于MS3组,差异无统计学意义(P>0.05)。Pearson相关分析中表现出血浆醛固酮与BMI、WC、TG、FPG、FINS、HOMA-IR、DBP呈正相关,与HDL-C呈负相关。多元回归分析表明,HOMA-IR是MS患者血浆醛固酮水平独立影响因素(β=0.12,P<0.005)。结论MS患者血浆醛固酮水平增高,增高的醛固酮与HOMA-IR和MS大部分组份相关。Objective To investigate the insulin resistance in patients with metabolic syndrome and the relationship between the plasma aldosterone levels.Methods 93 patients with MS,according to the different composition as MS,MS MS with type 2 diabetes and hypertension group,MS line all the subjects are 3 h oral glucose tolerance test and lie stand aldosterone test,and calculation formula of the steady-state model index of insulin resistance(HOMA IR).Results In ALD,INSAUS,HOMA-IR and HOMA-IR more than 2.18 indicators,the number of patients with MS had a higher proportion of each sub group,and were higher than the control group(P<0.05).In addition,the proportion of all indexes in MS1 group was higher than that in group MS2 and group MS3(P<0.05).The MS2 group accounted for slightly higher than that of MS3 group,the difference was not statistically significant(P>0.05).Pearson correlation analysis showed that bleeding,aldosterone and BMI,WC,TG,FPG,FINS,HOMA-IR,DBP were positively related,and negatively correlated with HDL-C.Multiple regression analysis showed that HOMA-IR was the independent factor of plasma aldosterone level in patients with MS(β=0.12,P<0.005).Conclusion The level of plasma aldosterone increased in MS patients,and the increased aldosterone was related to the majority of the components of HOMA-IR and MS.
