摘　　要：目的了解某三甲医院静脉用药调配中心不合理用药医嘱情况,提高临床合理用药水平。方法整理该三甲医院静脉用药调配中心2017年1—12月审方药师997 748条工作日志,对不合理医嘱进行统计分析。结果 997 748条医嘱中,共审核记录499条不合理医嘱(占总医嘱数的0.05%)。不合理原因主要涉及医嘱录入错误、给药剂量不合理、药物配置浓度不合理、溶媒选择不合理、重复用药、给药频次不合理、配伍不合理等。结论通过静脉用药调配中心审方药师对不合理医嘱的审核及干预,可有效减少临床不合理用药,提高药物治疗水平,保障患者用药安全。Objective To know the doctor’s orders of irrational medication in the pharmacy intravenous admixture service in some AAA hospital thus improving the clinical rational medication level.Methods The working logs of audit pharmacists in the pharmacy intravenous 997 748 admixture service in some AAA hospital from January to December 2017 were sorted up,and the irrational doctor’s signs were statistically analyzed.Results Of 997 748 doctor’s orders,499 irrational doctor’s orders were examined and recorded(accounting for 0.05%in total doctor’s orders),and the major irrational causes included the errors of medical advice entry,irrational administration dose,irrational drug dispensary concentration,irrational solvent selection,repeated medication,irrational medication frequency and irrational compatibility.Conclusion The examination and intervention of audit pharmacists in the irrational doctor’s orders in the pharmacy intravenous admixture service center can effectively reduce the irrational medication,improve the drug treatment level and ensure the medication safety of patients.
