摘　　要：目的:探讨白介素-17(IL-17)和基质金属蛋白酶-13(MMP-13)在多发性骨髓瘤(MM)骨髓活检组织中的表达及临床意义;方法:采用免疫组织化学技术检测47例MM患者及10例正常人骨髓活检组织中IL-17及MMP-13的表达。结果:MM组患者IL-17^+及MMP-13^+表达的平均光密度值均明显高于对照组(P<0.01)。MM组中骨病组(MBD组)IL-17^+及MMP-13^+表达的平均光密度值均明显高于无MBD组(P<0.01)。MM组NF-κB-p65^+表达率明显高于对照组(P<0.05)。在MM骨髓活检组织中IL-17与MMP-13的表达呈正相关(r=0.514)。治疗无效患者IL-17及MMP-13水平均明显高于有效患者(P<0.01)。结论:IL-17及MMP-13在MM中的表达明显高于对照组,且IL-17及MMP-13与MBD密切相关,这可能是通过NF-κB途径参与MBD的发病,同时IL-17及MMP-13可能作为评价治疗效果的潜在指标。Objective:To investigate the expression of interleukin-17(IL-17)and matrix metalloproteinase-13(MMP-13)in bone marrow biopsy of patients with multiple myeloma(MM)and its clicnical siginficonce.Methods:The expressions of IL-17 and MMP-13 in bone marrow biopsy of 47 patients with MM and 10 normal persons as controls were determined with immunohistochemical method.Results:The mean optical density of IL-17+and MMP-13+in MM group was significantly higher than that in control group(P<0.01).The mean optical density of IL-17+and MMP-13+positive expression in osteolytic lesions of MM group was significantly higher than that in patients without osteolytic lesions.The expression of NF-κB-p65+in MM group was significantly higher than that in control group(P<0.05);however the IL-17 level positively correlated with MMP-13 expression level(r=0.514)in MM group.The levels of IL-17 and MMP-13 in patients with ineffective treatment were very significantly higher than those in patients with effective trealment(P<0.01).Conclusion:The expression of IL-17 and MMP-13 in MM patients is significantly higher than that in the control group,and the expression of IL-17 and MMP-13 closely relate with MBD,which may be involved in the pathogenesis of MBD through NF-κB pathway,while the IL-17 and MMP-13 may serve as potential indicator for evaluation of therapeutic efficacy.
