摘　　要：目的通过颞骨高分瓣CT三维重建咽鼓管影像,探讨分泌性中耳炎(SOM)的咽鼓管的特征。方法收集SOM患者40例(63耳)及对照者18例(36耳),所有被试均行多层螺旋CT检查、多平面重组重建及图像处理,分析静态及Valsalva状态下软骨段开放长度,开放比例,咽鼓管圆枕厚度及下鼻甲距圆枕距离指标。结果与正常对照者相比,SOM患者Valsalva后软骨开放长度较短和Valsalva后软骨开放比例较小,差异有统计学意义。另外,SOM组的圆枕厚度较对照组小(P <0.001),下鼻甲距圆枕距离显著比对照组短(P=0.031)。但两组间全段软骨长度、静态下软骨开放长度以及静态下软骨开放比例无统计学差异。结论 SOM患者咽鼓管开放功能不良及咽鼓管圆枕结构存在异常。Objective To study the characteristics of the eustachian tube of the secretory otitis media(SOM)by using the high-resolution computed tomography(HRCT)3D reconstruction of the temporal bone.Methods Collect SOM patients 40 cases(63 ears)and 18 patients(36 ears)controls.Multislice spiral CT examination,multiplane reconstruction and image processing were performed in all cases,Analysis of the open length,open ratio of the cartilage,the thickness of the torus and the distance between the inferior turbinate and the torus of the static and Valsalva states.Results Compared to control group.The open length and proportion of cartilage in the secretory otitis media were less than that in normal group,Difference between the two groups has statistical significance.In addition,the torus thickness of SOM group was smaller than that of the control group(P<0.001),The inferior turbinate distance was significantly shorter than the control group(P=0.031).However,there was no statistically significant difference between the total length of cartilage between the two groups,the open length of the static lower cartilages,and the open ratio of the resting cartilage.Conclusion There is an abnormality in the open degree and torus structure of the eustachian tube of the secretory otitis media,which may be related to the disease.
