摘 要:目的:探讨大鼠切口疼痛对舒芬太尼(sufentanil)引起的躯体依赖性和精神依赖性的影响。方法:(1)躯体依赖性实验:SD雄性大鼠24只,按随机数字表分为4组:假手术组(C组)、疼痛模型组(P组)、假手术+舒芬太尼组(CS组)、疼痛模型+舒芬太尼组(PS组),每组6只。P组、PS组采用Brennan法制作趾部切口疼痛模型,观察疼痛行为1 h,记录累积疼痛评分;按照递增剂量法连续7天腹腔注射(ip.)舒芬太尼(5~30μg·kg^(-1),2次/日)建立躯体依赖模型,第7天(ip.)纳洛酮,观察20 min内大鼠戒断症状和体重变化。(2)精神依赖性实验:另24只大鼠分组、趾部切口疼痛模型建立及观察指标同躯体依赖性实验;按恒定剂量法连续7天交替(ip.)舒芬太尼(20μg·kg^(-1),1次/日)或生理盐水,诱导大鼠舒芬太尼的条件性位置偏爱(conditioned place preference,CPP)建立精神依赖模型,行CPP测试并用ELISA法测大鼠脑组织多巴胺(Dopamine,DA)含量。结果:(1)躯体依赖性实验:P组与PS组的累积疼痛评分显著高于C组和CS组(P<0.01);CS组和PS组的戒断评分显著高于C组和P组(P<0.01),同时,CS组和PS组的体重下降值也显著大于C组和P组(P<0.05);其余各组差异无统计学意义。(2)精神依赖性实验:P组和PS组的累积疼痛评分显著高于C组和CS组(P<0.01);同时,CS组和PS组的CPP值显著高于C组和P组(P<0.01),CS组和PS组脑组织DA含量也显著高于C组和P组(P<0.01,P<0.05);与CS组相比,PS组CPP值以及脑组织DA量都较低,差异有统计学意义(P<0.01);其余各组CPP值、脑组织DA量差异无统计学意义。结论:术后切口疼痛可以减弱舒芬太尼精神依赖性的形成,但不影响其躯体依赖性的形成。Objective:To investigate the effect of postoperative incision pain on physical and psychological dependence induced by Sufentanil in rats.Methods:(1)Physical dependence:Twenty-four SD rats were randomly divided into four groups(n=6):Sham operation group(C),Pain model group(P),Sham operation+Sufentanil group(CS),Pain model+Sufentanil group(PS).The plantar postoperative pain model was made by Brennan's method in group P and PS.The pain behavior and cumulative score were recorded 1h after operation.Sufentanil was intraperitoneal injected(ip.)with gradually increasing doses for 7 days(from 5-30μg·kg-1,two times daily)to establish a physical dependence model.In the 7th afternoon,naloxone was used for inducing withdrawal syndrome,the withdrawal score and weight loss of rats were recorded.(2)Psychological dependence:Other twenty-four rats were randomly divided into four groups,the plantar postoperative pain model was established,the pain behavior and cumulative score were also observed,which were as same as the physical dependence experiment.According to the constant dose method,the Sufentanil Conditioned Place Preference(CPP)was induced by alternatively(ip.)Sufentanil(20μg·kg-1,once daily,8AM)and saline(2 PM)for 7 days in rats to establish psychological dependence models.The CPP test was performed and the content of dopamine(DA)in the brain was measured by ELISA.Results:(1)Physical dependence:The cumulative pain scores of groups P and PS were significantly higher than those of groups C and CS(P<0.01).Compared with group C and group P,the withdrawal scores and the weight loss of group CS and group PS were significantly increased(P<0.01).There was no significant difference in the withdrawal scores and weight loss among the other groups.(2)Psychological dependence:Compared with the group C and group CS,the cumulative pain scores of group P and group PS were significantly increased(P<0.01);The CPP values of groups CS and PS were signifiantly larger than these of groups C and P(P<0.01);The DA content of groups CS and PS
