左西孟旦对扩张型心肌病所致的终末期心力衰竭的疗效研究

摘　　要：目的探讨左西孟旦对扩张型心肌病(DCM)所致的终末期心力衰竭患者的疗效。方法将80例DCM所致的终末期心力衰竭患者随机分为对照组和试验组,每组各40例,对照组给予常规药物治疗,试验组在常规药物治疗基础上给予左西孟旦治疗,分别检测生化指标、B型利钠肽(BNP)水平、心脏超声、6min步行距离,观察有无药物不良反应。结果试验组患者BNP水平较对照组显著降低,6min步行距离显著增加,左心室射血分数(LVEF)和左心室短轴缩短率(LVFS)显著升高,而左室收缩末期内径(LVESD)和左室舒张末期内径(LVEDD)显著降低,差异均有统计学意义(P<0.05),且不良反应发生率小于对照组(P<0.05)。结论左西孟旦可以显著改善DCM所致的终末期心力衰竭,提高其心功能及运动耐量,且不良反应发生率较低。Objective To investigate the therapeutic effect of levosimendan on end-stage heart failure induced by dilated cardiomyopathy.Methods Eighty patients with end-stage heart failure induced by dilated cardiomyopathy were randomly divided into the control group and experimental group,40 cases in each group.The control group received conventional therapy,and the experimental group was given levosimendan treatment on the basis of conventional drug treatment.All patients were detected biochemical indicators,the level of brain natriuretic peptide(BNP),echocardiography,6 min walking distance and drug adverse reaction.Results Compared with the control group,the BNP levels of patients in the experimental group decreased significantly,6 min walking distance increased significantly,left ventricular ejection fraction and left ventricular fractional shortening increased significantly,while left ventricular end systolic diameter and left ventricular end diastolic diameter decreased significantly.There was less drug adverse reaction in the experimental group than in the control group.Conclusion Levosimendan can significantly improve symptoms of patients with end-stage heart failure induced by dilated cardiomyopathy,improve cardiac function and exercise tolerance,and the incidence of adverse reactions is low.
