桥本氏甲状腺炎患者血清TPOAb、TgAb水平与病情的相关性分析

摘　　要：目的探讨桥本氏甲状腺炎(HT)患者血清甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(TgAb)水平与病情的相关性。方法测定80例HT患者及50名体检健康者(正常对照组)血清TPOAb、TgAb水平,分别根据血清TPOAb、TgAb中位数将HT患者分为高TPOAb组(40例)、低TPOAb组(40例)和高TgAb组(40例)、低TgAb组(40例)。同时测定HT组血清甲状腺功能指标[促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT_3)、游离甲状腺素(FT_4)]、辅助性T细胞(Th)1细胞因子[γ-干扰素(IFN-γ)、白细胞介素(IL)2]、Th2细胞因子(IL-4、IL-13)水平。结果 HT组血清TPOAb、TgAb水平均高于正常对照组(P<0.05)。高TPOAb组、高TgAb组血清TSH水平分别高于低TPOAb组、低TgAb组,FT_4、FT_3水平分别低于低TPOAb组、低TgAb组,血清IFN-γ、IL-2水平分别高于低TPOAb组、低TgAb组,血清IL-4、IL-13水平分别低于低TPOAb组、低TgAb组(P<0.05)。结论 HT患者体内TPOAb、TgAb等抗体呈高表达,且其水平与甲状腺功能损伤程度、Th1/Th2免疫紊乱程度有关,可反映病情严重程度。Objective To investigate the correlations of serum anti-thyroid peroxidase antibody(TPOAb)and thyroid-globulin antibody(TgAb)levels in Hashimoto's thyroiditis(HT)patients with disease severity.Methods A total of 80 HT patients were enrolled as HT group,and 50 healthy subjects were enrolled as healthy control group.The levels of serum TPOAb and TgAb were determined.They were further classified into high TPOAb group(40 cases),low TPOAb group(40 cases),high TgAb group(40 cases)and low TgAb group(40 cases)according to TPOAb and TgAb medians.Serum thyroid function indices[thyroid-stimulating hormone(TSH),free triiodothyronine(FT3)and free thyroxine(FT4)],T helper cell(Th)1[interferon gamma(IFN-γ)and interleukin(IL)-2 and Th2(IL-4 and IL-13)]were determined in HT group.Results Serum TPOAb and TgAb levels in HT group were higher than those in healthy control group(P<0.05).Serum TSH in high TPOAb group and high TgAb group were higher than those in low TPOAb group and low TgAb group,and FT3 and FT4 were lower than those in low TPOAb group and low TgAb group.Serum IFN-γand IL-2 in high TPOAb group and high TgAb group were higher than those in low TPOAb group and low TgAb group.Serum IL-4 and IL-13 were lower than those in low TPOAb group and low TgAb group(P<0.05).Conclusions TPOAb and TgAb have high expressions in HT patients,and there are relations of TPOAb and TgAb levels with thyroid damage degree and Th1/Th2 immune disorder degree,which can reflect the severity of disease.
