达比加群酯在房颤患者长期抗凝中的安全性及有效性

摘　　要：目的探讨达比加群酯在国内非瓣膜性房颤患者长期应用的有效性及安全性。方法将住院的112例非瓣膜性房颤患者随机分为两组,一组为达比加群酯组58例,给予达比加群酯胶囊110 mgbid抗凝治疗;一组为华法林组54例,根据INR目标值2.0～3.0,个体化给予华法林片抗凝治疗。所有患者随访至少1年时间,观察两组的出血事件、血栓栓塞事件及全因死亡率情况,比较两组之间的是否存在差异性。结果华法林组发生出血事件5例(8.8%),达比加群酯组2例(3.4%),差异无统计学意义(P> 0.05),华法林组未发生血栓血栓事件,达比加群酯组发生1例(1.7%),差异无统计学意义(P> 0.05);华法林组全因死亡7例(12.3%),达比加群酯组3例(8.7%),差异无统计学意义(P> 0.05)。结论达比加群酯在非瓣膜性房颤患者长期抗凝治疗中的有效性及安全性与华法林相当。Objective To investigate the long-term effectiveness and safety of dabigatran in patients with non-valvular atrial fibrillation in China.Methods A total of 112 patients with nonvalvular atrial fibrillation who were hospitalized in our department were randomly divided into two groups.One group was given anticoagulant therapy with dabigatran 110 mg bid for 58 patients,and 54 patients in the warfar group were treated for anticoagu-lant therapy according to the target value of INR 2.0-3.0.All the patients were followed up for at least 1 years and observed bleeding events,thrombotic events and all-cause mortality,and then compared is there any difference between the two groups.Results In the event of bleeding,there were 5 cases(8.8%)in warfarin group and 2 cases(3.4%)in dabigatran group,with no significant difference(P>0.05).No thrombotic thrombosis occurred in warfarin group,One case(1.7%)occurred in the dabigatran group,with no statistical significance.Warfarin all-cause mortality in 7 cases(12.3%),dabigatran group 3 patients(8.7%),no significant difference(P>0.05).Conclusion The efficacy and safety of dabigatran in long-term anticoagulation in patients wit h non-valvular atrial fibrillation is comparable to warfarin.
