超声心动图评价室壁瘤对左心室功能影响研究

摘　　要：目的探讨超声心动图评价室壁瘤对左心室功能的影响。方法选取自2016年10月至2018年6月沈阳军区总医院心血管内科收治的急性心肌梗死合并室壁瘤形成的415例患者为研究对象,根据美国纽约心脏病学会分级(NYHA)将140例NYHAⅡ级患者分为A组,185例NYHAⅢ级患者为B组,90例NYHAⅣ级患者为C组。对3组患者进行超声心动图检查,测量并比较心功能参数及室壁瘤参数。结果 3组左心室舒张功能均不同程度减低,组间比较,差异无统计学意义(P> 0. 05)。与A组比较,C组患者的左心室舒张末内径、左心室舒张末容积、左心室收缩末容积及室壁瘤瘤体基底部横径、高度、面积、容积均显著升高,左心室射血分数明显降低,差异均有统计学意义(P <0. 05)。左心室射血分数与室壁瘤瘤体基底部横径、室壁瘤瘤体面积及室壁瘤瘤体容积均呈正相关。结论心肌梗死室壁瘤形成后左心功能与室壁瘤范围相关,超声心动图可精准测量心功能参数及室壁瘤范围,对后续临床治疗具有重要价值。Objective To investigate the effect of the range of aneurismal on left ventricular function by echocardiography.Methods A retrospective study was performed on 415 cases of patients with acute myocardial infarction complicated with ventricular aneurysm who were admitted from October 2016 to June 2018.Patients were divided into three groups according to NYHA classification:140 cases of patients with NYHA class II in Group A,185 cases of patients with NYHA class III in Group B and 90 cases of patients with NYHA class IV in Group C.Echocardiography was performed on three groups of patients to measure the cardiac function parameters and the ventricular wall parameters and compare them.Results The left ventricular diastolic function of the three groups was reduced to different degrees,while there was no significant difference between the groups(P>0.05).Compared with Group A,the left ventricular end-diastolic diameter,the left ventricular end-diastolic volume,the left ventricular end-systolic volume,and the rumen base of the ventrioma in Group C all increased significantly and the left ventricular ejection fraction reduced significantly,with statistically significant differences(P<0.05).The left ventricular ejection fraction was positively correlated with the basal cross-diameter,the tumor area and the volume of the tumor.Conclusion Left ventricular function after the formation of myocardial infarction is related to the range of ventricular wall tumor.Echocardiography can accurately measure the cardiac function parameters and the range of ventricular wall tumor,which is of great value for subsequent clinical treatment.
