股骨近端防旋髓内钉与锁定钢板内固定治疗股骨转子间骨折临床疗效对比分析

摘　　要：目的分析股骨近端防旋髓内钉与锁定钢板内固定治疗股骨转子间骨折的疗效。方法 40例股骨转子间骨折患者作为研究对象,根据治疗方法不同分为股骨近端防旋髓内钉(PFNA)组与动力髋螺钉(DHS)组,各20例。对比分析两组手术指标、术后髋关节功能恢复情况。结果 PFNA组手术时间(64.58±8.55)min、切口长度(4.05±2.01)cm、术中出血量(158.31±37.22)ml、住院时间(15.50±2.44)d、骨折愈合时间(10.01±1.05)周明显优于DHS组(75.32±8.11)min、(8.45±2.11)cm、(222.44±42.35)ml、(23.15±3.55)d、(12.58±2.95)周,差异有统计学意义(P<0.05)。PFNA组术后3、6、12个月髋关节Harris评分(66.58±5.85)、(75.36±6.52)、(85.22±7.58)分明显优于DHS组(60.05±3.21)、(70.12±5.66)、(76.43±6.12)分,差异有统计学意义(P<0.05)。结论采用PFNA内固定治疗老年股骨转子间骨折患者,疗效显著,不仅能改善手术相关指标,提高术后疗效,而且能提升患者的生活质量,值得临床大力推广。Objective To analyze the efficacy of proximal femoral nail anti rotation and locking plate internal fixation in the treatment of intertrochanteric fracture.Methods A total of 40 patients with intertrochanteric fracture were selected as research objects,they were divided into proximal femoral nail anti rotation(PFNA)group and dynamic hip screw(DHS)according to different treatment methods,20 cases in each group.The operative indexes and postoperative recovery of hip joint function between the two groups were compared and analyzed.Results The operative time,incision length,intraoperative blood loss,length of stay and fracture healing time in the PFNA group were(64.58±8.55)min,(4.05±2.01)cm,(158.31±37.22)ml,(15.50±2.44)d and(10.01±1.05)weeks respectively,which were significantly better than(75.32±8.11)min,(8.45±2.11)cm,(222.44±42.35)ml,(23.15±3.55)d and(12.58±2.95)weeks in the DHS group,the differences were statistically significant(P<0.05).The Harris scores of hip function in the PFNA group after 3,6,12 months of surgery were(66.58±5.85)scores,(75.36±6.52)scores and(85.22±7.58)scores respectively,which were significantly better than(60.05±3.21)scores,(70.12±5.66)scores and(76.43±6.12)scores in the DHS group,the differences were statistically significant(P<0.05).Conclusion The curative effect of PFNA internal fixation in the treatment of elderly patients with intertrochanteric fractures is significant,not only can improve the operative related indicators,improve the postoperative efficacy,but also improve the quality of life of patients,it is worthy of clinical promotion.
