摘　　要：目的分析颈动脉粥样硬化超声检查联合血脂检测在冠状动脉粥样硬化病变筛查中的价值。方法选择2012年5月至2017年5月我院接诊的冠状动脉粥样硬化病变患者140例作为观察对象,依据冠状动脉病变的支数分为单支病变组和多支病变组,每组70例;选择同年龄段体检行冠状动脉造影检查的健康者80例为对照组。比较三组研究对象的颈动脉内中膜厚度(IMT)、斑块积分、冠状动脉Gensini积分及血脂指标水平。结果三组对象的IMT、颈动脉斑块积分及冠状动脉Gensini积分比较表现为多支病变组>单支病变组>对照组,差异有统计学意义(P <0. 05);三组对象的LDL、TG水平比较表现为多支病变组>单支病变组>对照组,差异有统计学意义(P <0. 05)。结论颈动脉粥样硬化的超声表现与冠状动脉粥样硬化的严重程度密切相关,颈动脉粥样硬化患者的血脂水平与冠状动脉病变的严重程度密切相关,颈动脉粥样硬化超声检查联合血脂检测对临床筛查冠状动脉粥样硬化病变具有重要的参考价值。Objective To analyze the value of carotid atherosclerosis combined with blood lipid test in the screening of coronary atherosclerotic lesions.Methods 140 patients with coronary atherosclerotic lesions who were admitted to our hospital from May 2012 to May 2017 were selected,who were divided into single-vessel disease group and multi-vessel disease group according to the number of coronary artery lesions.70 cases in each group.80 healthy subjects who underwent coronary angiography in the same age group were selected as the control group.Carotid intima-media thickness(IMT),plaque score,coronary Gensini score,and blood lipid index were compared between the three groups.Results The IMT,carotid plaque scores and coronary Gensini scores of the three groups were compared in the multi-vessel disease group>single-vessel disease group>control group,the difference was statistically significant(P<0.05).The LDL and TG levels of the three groups were compared in the multi-vessel disease group>single-vessel disease group>control group,the difference was statistically significant(P<0.05).Conclusion Ultrasound findings of carotid atherosclerosis are closely related to the severity of coronary atherosclerosis.The level of blood lipid in patients with carotid atherosclerosis is closely related to the severity of coronary artery disease.Carotid atherosclerosis combined with blood lipid test Clinical screening for coronary atherosclerotic lesions has important reference value.
