环磷酰胺冲击联合激素治疗系统性红斑狼疮的临床效果分析

摘　　要：目的探讨环磷酰胺冲击联合激素治疗系统性红斑狼疮患者的临床效果。方法选取2016年3月至2017年5月在我院治疗的系统性红斑狼疮患者66例,随机分为对照组和观察组,每组33例。对照组患者给予激素治疗;观察组患者在对照组治疗的基础上给予环磷酰胺冲击治疗,两组患者均治疗12个月。检测比较两组患者治疗前后的免疫功能,比较两组患者的临床疗效和不良反应发生情况。结果治疗1周后,两组患者C4、Ig M、Ig G、Ig A水平均显著改善(P <0. 05),观察组患者的改善情况优于对照组,差异有统计学意义(P <0. 05)。疗程结束后,观察组患者的治疗总有效率(93. 94%)显著高于对照组(75. 76%),差异有统计学意义(P <0. 05)。治疗过程中,两组患者的不良反应发生率比较差异无统计学意义(P> 0. 05)。结论环磷酰胺冲击联合激素治疗系统性红斑狼疮患者,可明显改善免疫功能,显著提高临床治疗效果,安全可靠。Objective To investigate the clinical effects of cyclophosphamide shock combined with hormone therapy in patients with systemic lupus erythematosus.Methods 66 patients with systemic lupus erythematosus who were treated in our hospital from March 2016 to May 2017 were selected.They were randomly divided into control group and observation group,with 33 cases in each group.The control group were treated with hormone therapy,the observation group were given cyclophosphamide shock therapy on the basis of the control group.Both groups were treated for 12 months.The immune function was compared between the two groups before and after treatment.The clinical efficacy and adverse reactions of the two groups were compared.Results After 1 week of treatment,the levels of C3,C4,IgM,IgG,and IgA in the two groups were significantly improved(P<0.05).The improvement of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).At the end of the treatment period,the total effective rate of treatment in the observation group(93.94%)was significantly higher than that in the control group(75.76%),and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups during the treatment(P>0.05).Conclusion Cyclophosphamide shock combined with hormone therapy treat systemic lupus erythematosus patients can significantly improve whose immune function,clinical treatment,and it′s safe and reliable.
