摘　　要：目的探讨皮质抑素在慢性阻塞性肺疾病(慢阻肺)患者肺泡灌洗液中的表达及其意义。方法选择我院收治的慢性阻塞性肺病患者39例,按肺功能分为中度组(23例)和重度组(16例),选择因自觉胸痛、无吸烟史、自愿行支气管肺泡灌洗的健康者15例为对照组。对3组对象进行支气管肺泡灌洗,检测比较3组对象的血清C反应蛋白水平及肺泡灌洗液(BALF)中的皮质抑素水平;检测比较3组对象BALF中嗜中性粒细胞、巨噬细胞、嗜酸性粒细胞占细胞总数的百分比。结果 COPD中度组及重度组患者血清C反应蛋白水平和BALF中皮质抑素水平均显著高于对照组,COPD重度组患者血清C反应蛋白水平和BALF中皮质抑素水平均显著高于COPD中度组,差异有统计学意义(P <0. 05)。COPD中度组及重度组患者BALF嗜中性粒细胞百分比均显著高于对照组,巨噬细胞百分比均显著低于对照组,差异有统计学意义(P <0. 05)。结论皮质抑素在慢阻肺患者肺泡灌洗液中的表达显著增加,可能参与了患者气道炎症的反应过程。Objective To investigate the expression and significance of cortistatin in alveolar lavage fluid of patients with chronic obstructive pulmonary disease(COPD).Methods 39 patients with chronic obstructive pulmonary disease admitted to our hospital were divided into moderate group(23 cases)and severe group(16 cases)according to their lung function.15 healthy subjects who volunteered for bronchoalveolar lavage were selected as the control group,who had a history of chest pain and no smoking.Bronchoalveolar lavage was performed on 3 groups of subjects.Serum C-reactive protein levels and cortistat levels in bronchoalveolar lavage fluid(BALF)were compared between the three groups.The percentage of neutrophils,macrophages,and eosinophils in the total number of cells in the BALF of the three groups was compared.Results Serum C-reactive protein levels and cortisol levels in BALF were significantly higher in the moderate and severe groups than in the control group.Serum C-reactive protein levels and cortisol levels in BALF were significantly higher in the severe group than in the moderate group(P<0.05).The percentage of BALF neutrophils in the moderate and severe COPD patients was significantly higher than that in the control group,and the percentage of macrophages was significantly lower than that in the control group(P<0.05).Conclusion The expression of cortistatin in BALF of patients with chronic obstructive pulmonary disease is significantly increased,which may be involved in the process of airway inflammation in patients.
