摘　　要：目的探究低分子肝素治疗对老年重症肺炎患者动脉血气的影响。方法将我院收治的66例老年重症肺炎患者作为本次研究观察对象,并将其随机分为观察组与对照组,对照组33例,给予常规治疗;观察组33例,在常规治疗的基础上给予低分子肝素治疗。观察两组动脉血气改善情况。结果治疗前两组动脉血pH、氧合指数(PaO2/FiO2,OI)、二氧化碳分压(PaCO2)、乳酸(Lac)等动脉血气指标差异均无统计学意义(P> 0.05),治疗后两组血气指标均改善(P <0.05),且与对照组相比,观察组pH、PaO2/FiO2均较高,PaCO2、Lac均较低(P <0.05);治疗前两组APACHEⅡ评分差异无统计学意义(P> 0.05),治疗后两组APACHEⅡ评分均下降,且观察组评分低于同时期的对照组(P <0.05),两组数据差异具有统计学意义。两组均无不良反应发生(P> 0.05)。结论给予老年重症肺炎患者低分子肝素治疗成效显著,可改善患者动脉血气指标,改善氧合,且安全性较高,有效促进预后转归。Objective To explore the effect of low molecular heparin therapy on arterial blood gas in patients with severe pneumonia.Methods The 66 cases of elderly patients with severe pneumonia admitted to our hospital were selected as the subjects of this study,and they were randomly divided into observation group and control group,the 33 patients in the control group were given routine treatment.In the observation group,33 patients were treated with low molecular heparin on the basis of conventional treatment.The improvements of arterial blood gas in two groups were observed.Results Before treatment,there was no significant difference between the two groups in arterial blood pH,oxygenation index(PaO2/FiO2,OI),partial pressure of carbon dioxide(PaCO2),lactic acid(Lac),etc.(P>0.05).After treatment,the blood gas indexes of the two groups were improved(P<0.05),and compared with the control group,the pH,PaO2/FiO2 of the observation group were higher,the PaCO2 and Lac were lower(P<0.05).There was no significant difference in APACHE II score between the two groups before treatment(P>0.05).After treatment,the APACHE II score of the two groups decreased,and the score of the observation group was lower than that of the control group at the same time(P<0.05).No adverse reactions occurred in the two groups(P>0.05).Conclusion The treatment of low molecular heparin in elderly patients with severe pneumonia can significantly improve the arterial blood gas index,improve oxygenation,promote prognosis,and it's very safe.
