甲钴胺及叶酸对2型糖尿病伴高同型半胱氨酸血症患者认知功能的影响

摘　　要：目的观察甲钴胺、叶酸对2型糖尿病伴高同型半胱氨酸血症患者认知功能的影响。方法纳入高同型半胱氨酸(Hcy)血症伴轻度认知功能障碍的2型糖尿病患者105例,随机分成对照组55例,实验组50例。对照组常规治疗,实验组在常规治疗基础上加用甲钴胺片(每天3次,每次500 mg)及叶酸片(每天3次,每次5 mg)。在治疗前和治疗后6周、6个月,检测所有患者Hcy水平,同时采用简易智能精神状态检查量表(MMSE)评价其认知功能。结果对照组治疗前后Hcy水平及MMSE评分差异均无统计学意义(P> 0.05);实验组治疗后6周、6个月,Hcy水平均较治疗前下降(P <0.05),MMSE评分均提高(P <0.05)。实验组治疗前后MMSE评分与Hcy水平呈负相关。结论甲钴胺、叶酸联用可降低Hcy水平,并有助于改善2型糖尿病伴高同型半胱氨酸患者的认知功能。Objective To observe the effects of mecobalamine and folic acid on cognitive dysfunction in patiens with type 2 diabetes mellitus complicated with hyperhomocysteine.Methods 105 cases of type 2 diabetes with high homocysteine(Hcy)accompanied by mild cognitive dysfunction were included,and they were randomly divided into 55 cases in the control group and 50 cases in the experimental group.The control group was treated with conventional treatment,and the experimental group was treated with mecobalamine tablets(3 times daily,500 mg each time)and folic acid tablets(3 times daily,5 mg each time).Hcy levels were measured in all patients before and 6 weeks and 6 months after treatment,and cognitive function was evaluated using the simple mental state examination scale(MMSE).Results There was no statistically significant difference in Hcy level and MMSE score before and after treatment in the control group(P>0.05).After 6 weeks and 6 months of treatment,Hcy level in the experimental group was significantly lower than that before treatment(P<0.05),and MMSE score was significantly higher(P<0.05).Before and after treatment,MMSE score and Hcy level of the experimental group showed significant negative correlation.Conclusion The combination of mecobalamine and folic acid can significantly reduce the level of Hcy and improve the cognitive function of patients with type 2 diabetes mellitus.
