摘　　要：目的探讨防螺旋型股骨近端髓内钉(PFNA)手术方法治疗老年股骨粗隆间骨折的临床疗效。方法选取本院2015年1月～2016年11月在本院骨科收治的40例老年股骨粗隆间骨折患者,随机分为观察组与对照组,观察组采用PFNA治疗,对照组采用动力髋钉(DHS)手术方法治疗,对比分析两组患者疗效。结果观察组手术时间、术中出血量、术后引流量、术后卧床时间、手术切口长度均明显少于对照组(P<0.05),观察组治疗后Harris髋关节评分优良率为90%,对照组为70%,观察组疗效明显优于对照组,差异具有统计学意义(P<0.05),两组患者术前VAS评分比较差异无统计学意义,术后1个月、术后1年观察组VAS评分均明显优于对照组,差异具有统计学意义(P<0.05)。结论 PFNA手术方法治疗老年股骨粗隆间骨折术后卧床时间短、手术切口小、术中出血量小,治疗疗效相比较常规手术明显提高,有利于患者术后早期活动及康复,值得在临床中推广应用。Objective To investigate the clinical efficacy of anti-helical proximal femoral nail(PFNA)in the treatment of intertrochanteric fractures in the elderly.Methods Forty patients with senile intertrochanteric fractures admitted to our hospital from January 2015 to November 2016 were randomly divided into observation group and control group.The observation group was treated with PFNA and the control group was treated with dynamic hip nail.(DHS)surgical treatment,comparative analysis of the efficacy of the two groups of patients.Results The operation time,intraoperative blood loss,postoperative drainage volume,postoperative bed rest time and length of surgical incision in the observation group were significantly lower than those in the control group(P<0.05).The excellent rate of Harris hip score after treatment in the observation group was 90%.The control group was 70%.The effect of the observation group was significantly better than that of the control group.The difference was statistically significant(P<0.05).There was no significant difference in the preoperative VAS score between the two groups.One month after surgery and one year after surgery,the VAS scores of the observation group were significantly better than the control group,and the difference was statistically signifidoicant(P<0.05).Conclusion The PFNA operation method is suitable for the treatment of senile femoral intertrochanteric fractures.The postoperative bed time is short,the surgical incision is small,and the intraoperative blood loss is small.The therapeutic effect can be significantly improved compared with conventional surgery,which is beneficial to patients'early postoperative activities and rehabilitation.It is worthy of clinical Promote the application.
